A stair collapsed, forcing fire crews to access the building through the roof, the nursery being located on the 1st floor.  29th May 2012 France Staff endangered in shooting at Corsican hospital A 37-year old Corsican restaurateur was transferred to hospital in Nice on Monday evening after shots were fired at him as he lay in an intensive care unit in hospital in Bastia earlier on Monday. The shots were fired through the unit window from a 9mm handgun, endangering the patient and staff in the unit. The extent of their injuries was not known. 23rd May 2012 Austria Brake left on track caused derailment and foot amputation A railway worker's foot was amputated in a derailment accident on Monday morning in Marchtrenk, Upper Austria. Department of Labour investigation found that the company failed to isolate hazards with the machinery and that the installation of a robust isolation and lockout system and a high gate restricting access to the hazard area could have prevented the accident. 11th May 2012 Indonesia Russian promotional flight aircraft missing in West Java A Sukhoi Superjet-100 aircraft participating in the "Welcome Asia" promotional campaign was reported missing in West Java, the flight had left Jakarta's Halim Perdonakusuna International Airport for a 50-minute flight with 46 people on board. It was unclear whether he died from electrocution or multiple injuries sustained in the fall from the scaffold. 2nd May 2012 England Worker lost eye in Derbyshire farm quad bike accident A quad bike crash during the rounding up of sheep on land at Lodge Hill farm, Barton Blount, Derbyshire, led to a worker losing the sight of an eye, fracturing his jaw and sustaining pelvic and abdominal injuries. HSE inspectors found the bike to be in poor riding condition, with incorrectly inflated tyres, steering wear and poor braking. Initially it was speculated that a static charge had caused the blast but further investigation suggested that a reactor safety valve may have malfunctioned. New Zealand Aviation engineer was drawn into air intake At Blenheim District Court Safe Air Ltd has been fined $56,250(£27,367) plus $22,500(£10,947) in compensation over a fatal accident during engine testing at Woodbourne Airfield. Two aviation engineers were servicing  an engine mounted on an elevated work platform. One of them left the control room but as he passed the engine under test he was drawn into the air intake. He had failed to hold on to a handrail but investigation found that a proper guard was need which could have prevented the accident. The company pleaded guilty to a breach of Section 6 of the Health & Safety in Employment Act 1992. 31st May 2012 Belgium Heat sparks recycling facility fire It is speculated that the heat of the sun on piles of glass sparked a fire yesterday at the Cometsambre recycling facility at Obourg. Fire crews from Mons managed to control the blaze by early evening  and were damping down at the unit which also processes scrap cars. 30th May 2012 North Sea Yacht missing off Holland A major alert was announced yesterday over the safety of the Norwegian yacht Kohenji, missing in the North Sea during transit from Zeebrugge to Hanstholm in Denmark where the yacht and its 3-man crew were overdue yesterday. He was hospitalised with severe injuries.  28th May 2012 Spain Worker killed in fall from roof A maintenance worker of Macohe died in a 9-metre fall from the roof of a building at the Mirat fertiliser plant in Salamanca. 28th May 2012 England Herts toddler asphyxiated by blind cord A Hertfordshire Coroner has delivered a verdict of accidental death in respect of an accident on 5th January in a house in Markyate where a 2-year old boy standing on a stool at a window became entangled in a blind cord which led to asphyxiation. He sustained brain damage and died 5 days later in hospital. This was the 2nd such accident of its kind recently in Hertfordshire where a 2-year old girl died in December following a similar incident in August. 25th May 2012 Israel Fatal accident after concrete pour on Jerusalem site An accident occurred yesterday during a concrete pour at a newbuild site on Moshe Kol Street in the BeitHakerem district of Jerusalem which led to a partial collapse of the structure. One worker died in the accident and 2 others were injured. 24th May 2012 Pakistan Worker killed in oxygen cylinder filling explosion A gas cylinder exploded during the introduction of oxygen at the Sultan Gas Cylinder Company on the Korangi Industrial Area of Karachi. The blast caused several other cylinders to become unstable and explode, destroying part of the 2-storey building and killing a worker. Six others were injured, with at least one thought to be listed in critical condition. 23rd May 2012 England Worker's arm was severed in unguarded conveyor at Kent quarry At Dartford Magistrates' Court Pinden Limited has been fined £10K plus £11,506 in costs over an accident on December 20th 2010 when a 46-year old Latvian's arm became trapped in a moving conveyor system. He had apparently been trying to clear a blockage from the conveyor which transfers waste into a sorting shed at Pinden Quarry in Longfield. Investigation found there was no guarding to prevent access to dangerous moving parts on the conveyor and that the worker had received no training in clearing blockages safely. The company pleaded guilty to a breach of Section 2(1) of the Health & Safety at Work etc Act 1974. 22nd May 2012 England Search for missing Weymouth crabbers called off Portland Coastguard announced that the search for the 2 missing crabbers from the Purbeck Isle fishing boat which sank in 50 metres of water 10 miles off Portland has been called off, the skipper's body has already been recovered. 21st May 2012 Lebanon Crane overturns at port site A crane became unstable and overturned on a site in Jiyye at the weekend, killing a Syrian worker. 21st May 2012 France Flotsam holes Basque gillnetter The gillnetter Haura which fished out of Bayonne sank 20 miles off Mimizan, Landes, yesterday morning after striking floating material which caused substantial water ingress. The crew issued a mayday and took to their life raft, the Santa Rita II fishing boat responded and safely rescued the crew within an hour of the mayday. 18th May 2012 The Netherlands Barge/yacht collision off Zeeland Two yacht sailors were rescued from the water 4 miles off Bruinisse in Zeeland after their boat was in collision with a barge. The man managed to jump clear after the impact but the woman who was below deck had more difficulty getting to the surface. The boat's dog was lost in the accident. 18th May 2012 England Cumbrian injured in fall through roof A man is listed in critical condition in Newcastle's Royal Victoria Infirmary after falling through a fragile roof at premises in Langrigg, 20 miles southwest of Carlisle. 17th May 2012 France Fall from scaffold in Toulouse A construction worker has been hospitalised in Toulouse with severe head injuries and a considerable loss of blood after falling from a scaffold at the Empalot premises on rue des Saules in Toulouse. 17th May 2012 Germany Unloading crush accident at Düsseldorf fair site A worker was listed in life-threatening condition with severe chest injuries in hospital in Duisburg last night following an accident earlier on Tuesday in the Oberkassel park in Düsseldorf when he was struck by a pay booth being unloaded at the site of a fairground. 16th May 2012 England Gas fitter sentenced over boiler accident At Ipswich Crown Court a Norwich gas fitter received an 8-month suspended prison sentence over an accident in which an explosion occurred at a home in Wrentham, Suffolk, where he had been installing a boiler. A connection between an emergency control valve on the boiler and a pipe was improperly secured, causing gas to leak through the property, and when the resident ignited a kettle an explosion occurred, inflicting burns to her and a child. The gas fitter admitted breaching Regs 6(1) and 7(1) of the Gas Safety (Installation & Use) Regs 1998. He was also required to pay £10K costs and carry out 250 hours community service. 16th May 2012 Canada Arrest over murder of Alberta carer A 61-year old carer at the Marler Supported Independent Living Program in Camrose, Edmonton, was found dead at the premises, police are understood to be charging a 17-year old male with second degree murder. An autopsy of the carer is scheduled for today(Tuesday). 15th May 2012 Uruguay Worker killed guiding lorry in Montevideo A construction worker has died in a transport accident on a site in the port area of Montevideo where a lorry he was guiding skidded out of control. 14th May 2012 Panama Joiner died in fall at Hydro site Mitradel( Ministerio de Trabajo y Desarrollo Laboral) has issued a temporary suspension of work on the Baitun Bajo Mimo hydrolectric project of CILSA Panama in Renacimiento, Chiriqui, where a subcontract joiner died in a fall of several metres in an accident involving a concrete pump in the lower section of the dam. 14th May 2012 Rep Ireland Discarded syringe in bedding pricked hotel guest At the Circuit Civil Court in Dublin a young woman was awarded €17,500 (£14,023) in respect of an accident at the Castleknock Travel Lodge in Dublin on June 6th 2009 when she was pricked in the arm by a syringe needle discarded in bedding. It is alleged that the driver of the Fecha Amarilla coach was negligent in attempt to get over the crossing. 4th May 2012 Canada Accident during unloading from lorry to forklift The Ministry of Labour in Ontario is investigating an accident yesterday in which a worker sustained a serious foot injury when large metal tables became unstable and fell during unloading from a lorry trailer on to a forklift at a site in Barrie. 4th May 2012 Oman 3 road workers killed by car at work site Three road workers were killed at a highway work site in Al Aliya village in Al Sharqiyah when a vehicle travelling from Al Jardaa to Mehlah failed to take a bend on the road. It was unclear if the road site had adequate signage warning of a traffic hazard. 3rd May 2012 Australia Lorry driver crushed in scissors lift accident A crush accident has claimed the life of a lorry driver during the unloading of material at a site in Preston, North Melbourne. England Lancs coroner opines care home death fire was deliberate At Fleetwood Magistrates' Court a coroner delivered a verdict of unlawful killing in respect of the death of a 96-year old female resident of Cleveleys Park Rest Home whose bed was allegedly set on fire on the 18th of October 2010. She died in hospital in Blackpool the following day. The coroner indicated that he was "entirely satisfied the fire was started deliberately" and took comfort that it "was not a closed case." Lancashire Constabulary has the case on file as an ongoing murder inquiry, 2 home staff members held initially on suspicion of murder were released as there was insufficient evidence to charge either woman. 31st May 2012 England Fire at Shropshire car repair unit Fire crews took 3 hours to control a blaze yesterday at the J A Barnett Accident Repair centre on Heath Hill Industrial Estate in Dawley, Telford, where most of the roof collapsed and concern was expressed over the risk of a gas cylinder explosion. 30th May 2012 England Explosion at Lancs mill Two people were injured when an explosion occurred at Stansfield Mill, Littleborough, a silo containing solvents was ablaze with lpg gases and bitumen tanks posing hazards for firemen at the locus. 29th May 2012 Rep Ireland Cavan rally car hits spectators A man and a woman died when a car participating in the Cavan Stages Rally, organised by the Cavan Motor Club, left the road and struck spectators in Bailieborough. Seven others were injured, it is believed that the dead man was a photographer. 28th May 2012 England Divers drift in choppy seas Brixham Coastguard co-ordinated the rescue of 2 divers who had drifted a mile and a half from their dive boat off Wembury, Plymouth. Sea conditions were choppy at the time. 28th May 2012 Scotland Furnace fire at Sandvik in Perth A technical malfunction led to a fire in a furnace yesterday at the Sandvik premises on the Inveralmond Industrial Estate in Perth where the company produces silicon carbide heating elements. Staff attempted to control the blaze themselves before evacuating the building and calling the fire service. No injuries were reported, fire crews continued to damp down the locus until early evening. 25th May 2012 Belgium Worker crushed under forage harvester A 39-year old Czech lorry driver was killed in a crush accident under a forage harvester at the agricultural premises of Vandelanotte on Oude Gentweg, Torhout. The driver has responsibility for the removal of the vehicle's wheels prior to the machinery being unloaded, it is speculated that a jacking device failed. 24th May 2012 Italy Boat cleaners unconscious after confined space accident Two workers were admitted to Sant'Andrea Hospital in La Spezia after being found unconscious in an oxygen deficient atmosphere in a large yacht at Fincantieri's Muggiano Shipyard in La Spezia. They are employees of Sepor engaged in cleaning and degassing boats. Medics were being cautiously optimistic last night of their survival prospects. 23rd May 2012 Canada Reversing lorry hits Saskatoon worker An investigation was launched yesterday into an industrial accident on 45th Street in Saskatoon where a man was struck by a gravel-laden lorry, the victim's condition is not life-threatening. 23rd May 2012 England Heritage railway worker crushed between trains The Rail Accident Investigation Branch is investigating a crush fatality which occurred yesterday at Grosmont Station, the terminus of the North Yorks Moors Railway, where a worker became pinned between two trains. 22nd May 2012 Northern Ireland Northwest 200 biker killed in Portstewart A 35-year old Scottish biker of the Spitlath Redmond Racing Team died in an accident at the weekend in Portstewart while participating in the Northwest 200 motorcycle race. 21st May 2012 England Coils stored on roll end fell and killed warehouseman At Grimsby Crown Court ABC(Grimbsby)Ltd was fined £25K plus £20K costs over a fatal accident in its warehouse on Henderson Quay, Immingham Docks, where a worker was crushed by unstable banded coils. On 27th January 2010, a warehouseman was attempting to repair torn packaging and was standing in between the coils when they became unstable and fell. They were in batches of 4 or 5, each weighing 1 tonne, and were stored on roll end. Investigation noted that narrow banded coils stored on roll end can be unstable and it was found that employees had not received training in their safe handling and storage. The company pleaded guilty to breaching Section 2(1) of the Health & Safety at Work etc Act 1974 and Regulation 3(1) of the Management of Health & Safety Regulations 1999. 18th May 2012 Scotland Part-blocked chimney led to CO death At Haddington Sheriff Court an East Lothian builder was fined £15K over a carbon monoxide accident at a house in Gifford where a 60-year old woman died from carbon monoxide poisoning after he had rebuilt a chimney. A failure to clear debris from the chimney led to a CO build-up in the house from the fire, he pleaded guilty to breaching Sections 3 & 33(1) of the Health & Safety at Work etc Act 1974. 17th May 2012 England Woman crushed under tractor trailer in Lincs field The HSE has been advised of a fatal accident which occurred in a field in Leverton, Lincolnshire, yesterday morning where a female farm worker in her 40's died when she became trapped under a trailer being drawn by a tractor. 16th May 2012 Italy Fall from scaffold at demolition site A 56-year old Albanian worker was knocked unconscious in a fall from a scaffold on Monday morning in Forli. He was using a pneumatic drill during the demolition of a wall and he apparently lost his balance as he was handing the drill to a colleague. His condition is listed as not life-threatening. 15th May 2012 Scotland 2 dead in Ben More microlight crash Two people died when a microlight aircraft crashed on the slopes of Ben More on Saturday, an investigation into the accident is ongoing. 14th May 2012 Northern Ireland Unstable raised pallet led to teen's fall At Omagh Crown Court Reid Engineering (Cookstown) Ltd was fined £17,500 over an accident in which a 17-year old worker was injured when he fell 8 metres from a raised pallet used for access at premises in Lisnaskea, County Fermanagh. Four workers were on the pallet which lacked handrails and protective edging and had no means of being secured to the telescopic materials handler. The company pleaded guilty to breaching Article 4 of the Health & Safety at Work (Northern Ireland) Order 1978 and Regulation 6 of the Work at Height Regulations (Northern Ireland) 2005. 14th May 2012 Scotland Super Pumas suspended pending safety checks The Bond Aviation Group has temporarily grounded its fleet of EC225 Super Prima helicopters pending safety investigation following the controlled ditching of an aircraft into the North Sea 30 miles east of Aberdeen yesterday. All 14 people on board were safely rescued from their life raft, the flight from Aberdeen had been making for rigs in the Jasmine Field when a low oil pressure warning was indicated.  11th May 2012 Northern Ireland Woman and baby fall overboard from ferry A woman passenger was rescued from Belfast Lough yesterday evening after falling from the Stena Super Fast VII ferry as it approached Belfast Harbour. The woman was admitted to the Royal Victoria Hospital in hypothermic condition. It was also believed that a 9-day old baby had fallen into the water and was missing, Bangor Lifeboat was attending the incident. 10th May 2012 Northern Ireland Record fine over piggery accident A record fine of £187,500 plus £13K in costs was levied against JMW Farms at Belfast Crown Court in respect of a fatal accident at its pig farm in Tynan, County Armagh, on 15th November 2010 when a 45-year old worker died when a 200kg bin fell from a forklift truck at a meal mixing plant. He had been cleaning the bin from an adjacent position on a bale compactor, and as the bin was moved he jumped on to the side of it, but instability caused the bin and worker to fall to the ground. Investigation found that the bin was inadequately attached, with the forks not fitting the bin's sleeves. The forklift was replacing another machine undergoing servicing. 9th May 2012 Australia Worker left partially disabled by fall Downer EDI Works was fined $187,500(£117,366) over an accident in May 2009 in which a worker was severely injured in a fall into a pit during cleaning work on a machine used in the preparation of asphalt. He became crushed between an incline conveyor belt and a drum roller, sustaining fractures and injuries which left him a partial tetraplegic. Investigation found that there was a lack of guard railing around the machinery. 9th May 2012 England Diver dies during training exercise The HSE has been advised of a fatal accident at Stoney Cove in Stoney Stanton, Leicestershire, where a diver taking instruction from an Essex diving team died following an accident during an advanced training exercise at the flooded quarry. 8th May 2012 Romania Boy drowns herding cattle A 10-year old boy has drowned while apparently herding cattle on a farm in Bistrita, Valcea County. Some of the herd had slipped into a swollen river and he himself fell in, losing his balance on wet rocks. 8th May 2012 Germany Driver pinned between lorry and wall A 48-year old lorry driver was hospitalised in Berlin yesterday with crush injuries after becoming pinned between his vehicle and a house wall in Neuzelle, Oder Spree. He had apparently alighted from the lorry which began to roll backwards and became pinned as he south to remount the vehicle. 4th May 2012 Azerbaijan Unstable timber load hits worker at Eurovision Song venue A 23-year old European worker has been hospitalised in Baku with unspecified injuries after being struck a by a pile of timber sections which became unstable at the Crystal Hall in the Eurovision Arena where the stage is being prepared for the forthcoming Eurovision Song Contest. 3rd May 2012 England Inquest opens into woman's crush death at Cambridge works The HSE has been investigating a recent fatal accident at RGE Engineering in Cambridge where a woman died after machinery fell on her, an inquest into the accident opens today (Wednesday) in Huntingdon. 2nd May 2012 England Shipyard fined over apprentice's fall At Truro Magistrates' Court  Pendennis Shipyard of Falmouth was fined £6K plus £6,288 in costs over an accident in which an apprentice worker sustained knee injuries in a 2-metre fall from a scaffold. He had been removing plastic covering over a boat being painted when a scaffold board became unstable. Inspectors found an absence of a risk assessment and safe working method for workers, and a lack of information, training, instruction and supervision. 2nd May 2012 England Beds glider pilot killed The Air Accident Investigation Branch has been advised of a fatal accident in Bedfordshire yesterday after a glider pilot died in a crash in Eaton Bray. 1st May 2012 England Fire destroys Falmouth hotel Fire broke out on the 3rd floor of the 4-storey Falmouth Beach Hotel yesterday, completely destroying the 120-bed hotel in Gyllyngvase, Falmouth. Best Western has been operating the hotel for the last 12 years, no injuries were reported and it appeared that all staff and guests have been accounted for. 1st May 2012
