A 31-year old Mallorcan worker at the Son Reus waste incinerator
facility in Palma was hospitalised in critical condition in the burns
unit of a hospital in Barcelona after 50% of his body was exposed to
steam burns in an accident on Wednesday morning. He was admitted to
the local Son Espases hospital on the island but the critical nature
of his injuries necessitated specialist treatment in Barcelona.

Staff endangered in shooting at Corsican hospital
A 37-year old Corsican restaurateur was transferred to hospital in
Nice on Monday evening after shots were fired at him as he lay in an
intensive care unit in hospital in Bastia earlier on Monday. The shots
were fired through the unit window from a 9mm handgun, endangering the
patient and staff in the unit.
The patient had been hospitalised following an earlier shooting
incident and is understood to be known to French police from a
previous extortion case.

Hakkatoge tunnel in Niigata where a gas explosion had occurred about a

The Dersu Uzala trawler responded to a mayday and rescued all 91 crew
of the trawler Captain Bolsunovsky which sank 40 miles off the
Kamchatka peninsula. Both trawlers fish out of Kholinsk, the stricken
boat was holed by ice but managed to remain afloat for almost an hour
after issuing the mayday.

At Napier District Court Pan Pac Forest Products were fined
$39K(£18,972) plus $7,500(£3,649) costs over an accident in which a

Department of Labour investigation found that the company failed to
isolate hazards with the machinery and that the installation of a

Worker lost eye in Derbyshire farm quad bike accident
A quad bike crash during the rounding up of sheep on land at Lodge
Hill farm, Barton Blount, Derbyshire, led to a worker losing the sight
of an eye, fracturing his jaw and sustaining pelvic and abdominal
injuries.
HSE inspectors found the bike to be in poor riding condition, with
incorrectly inflated tyres, steering wear and poor braking.
At Southern Derbyshire Magistrates' Court JD & RL Spalton admitted
that the bike was poorly maintained and a failure to supply a crash
helmet which could have mitigated the injuries. A fine of £2K was
given plus £2K costs for breaching Reg 4(1) of the Personal Protective
Equipment at Work Regs 1992 and Reg 5 of the Provision and Use of Work
Equipment Regs 1998.

4 workers burned alive in chemical plant fire
Four workers were burned alive yesterday following an explosion at
Dashmesh Medicare, a chemical plant some 25 miles from Chandigarh.
Fifteen others were injured and 8 are believed to have 50% burns.
Initially it was speculated that a static charge had caused the blast
but further investigation suggested that a reactor safety valve may
have malfunctioned. Police are expected to lay charges under Section
304 of the Indian Penal Code( causing death by negligence) against the
factory owner and management.

Two aviation engineers were servicing an engine mounted on an
elevated work platform. One of them left the control room but as he
passed the engine under test he was drawn into the air intake. He had
failed to hold on to a handrail but investigation found that a proper
guard was need which could have prevented the accident. The company
pleaded guilty to a breach of Section 6 of the Health & Safety in
Employment Act 1992.

It is speculated that the heat of the sun on piles of glass sparked a
fire yesterday at the Cometsambre recycling facility at Obourg. Fire
crews from Mons managed to control the blaze by early evening
and were damping down at the unit which also processes scrap cars.

Yacht missing off Holland
A major alert was announced yesterday over the safety of the Norwegian
yacht Kohenji, missing in the North Sea during transit from Zeebrugge
to Hanstholm in Denmark where the yacht and its 3-man crew were
overdue yesterday. No radio contact was possible with the yacht and no
mayday has been received by the Belgian or Dutch coastguard.

Herts toddler asphyxiated by blind cord
A Hertfordshire Coroner has delivered a verdict of accidental death in
respect of an accident on 5th January in a house in Markyate where a
2-year old boy standing on a stool at a window became entangled in a
blind cord which led to asphyxiation. He sustained brain damage and
died 5 days later in hospital.
This was the 2nd such accident of its kind recently in Hertfordshire
where a 2-year old girl died in December following a similar incident

Worker killed in oxygen cylinder filling explosion
A gas cylinder exploded during the introduction of oxygen at the
Sultan Gas Cylinder Company on the Korangi Industrial Area of Karachi.
The blast caused several other cylinders to become unstable and
explode, destroying part of the 2-storey building and killing a
worker. Six others were injured, with at least one thought to be
listed in critical condition.

Worker's arm was severed in unguarded conveyor at Kent quarry
At Dartford Magistrates' Court Pinden Limited has been fined £10K plus
£11,506 in costs over an accident on December 20th 2010 when a 46-year
old Latvian's arm became trapped in a moving conveyor system. He had
apparently been trying to clear a blockage from the conveyor which
transfers waste into a sorting shed at Pinden Quarry in Longfield.
Investigation found there was no guarding to prevent access to
dangerous moving parts on the conveyor and that the worker had
received no training in clearing blockages safely. The company pleaded
guilty to a breach of Section 2(1) of the Health & Safety at Work etc

safely rescued the crew within an hour of the mayday.

Gas fitter sentenced over boiler accident
At Ipswich Crown Court a Norwich gas fitter received an 8-month
suspended prison sentence over an accident in which an explosion
occurred at a home in Wrentham, Suffolk, where he had been installing
a boiler. A connection between an emergency control valve on the
boiler and a pipe was improperly secured, causing gas to leak through
the property, and when the resident ignited a kettle an explosion
occurred, inflicting burns to her and a child.
The gas fitter admitted breaching Regs 6(1) and 7(1) of the Gas Safety
(Installation & Use) Regs 1998. He was also required to pay £10K costs
and carry out 250 hours community service.

Lancs coroner opines care home death fire was deliberate
At Fleetwood Magistrates' Court a
coroner delivered a verdict of unlawful killing in respect of the
death of a 96-year old female resident of Cleveleys Park Rest Home
whose bed was allegedly set on fire on the 18th of October 2010. She
died in hospital in Blackpool the following day. The coroner indicated
that he was "entirely satisfied the fire was started deliberately" and
took comfort that it "was not a closed case."
Lancashire Constabulary has the case on file as an ongoing murder
inquiry, 2 home staff members held initially on suspicion of murder
were released as there was insufficient evidence to charge either

A technical malfunction led to a fire in a furnace yesterday at the

continued to damp down the locus until early evening.

Coils stored on roll end fell and killed warehouseman
At Grimsby Crown Court ABC(Grimbsby)Ltd was fined £25K plus £20K costs
over a fatal accident in its warehouse on Henderson Quay, Immingham
Docks, where a worker was crushed by unstable banded coils.
On 27th January 2010, a warehouseman was attempting to repair torn
packaging and was standing in between the coils when they became
unstable and fell. They were in batches of 4 or 5, each weighing 1
tonne, and were stored on roll end.
Investigation noted that narrow banded coils stored on roll end can be
unstable and it was found that employees had not received training in
their safe handling and storage. The company pleaded guilty to
breaching Section 2(1) of the Health & Safety at Work etc Act 1974 and
Regulation 3(1) of the Management of Health & Safety Regulations 1999.

At Haddington Sheriff Court an East Lothian builder was fined £15K
over a carbon monoxide accident at a house in Gifford where a 60-year
old woman died from carbon monoxide poisoning after he had rebuilt a
chimney. A failure to clear debris from the chimney led to a CO
build-up in the house from the fire, he pleaded guilty to breaching
Sections 3 & 33(1) of the Health & Safety at Work etc Act 1974.

At Omagh Crown Court Reid Engineering (Cookstown) Ltd was fined
£17,500 over an accident in which a 17-year old worker was injured
when he fell 8 metres from a raised pallet used for access at premises
in Lisnaskea, County Fermanagh.
Four workers were on the pallet which lacked handrails and protective
edging and had no means of being secured to the telescopic materials
handler. The company pleaded guilty to breaching Article 4 of the
Health & Safety at Work (Northern Ireland) Order 1978 and Regulation 6
of the Work at Height Regulations (Northern Ireland) 2005.

Super Pumas suspended pending safety checks
The Bond Aviation Group has temporarily grounded its fleet of EC225
Super Prima helicopters pending safety investigation following the
controlled ditching of an aircraft into the North Sea 30 miles east of
Aberdeen yesterday.
All 14 people on board were safely rescued from their life raft, the
flight from Aberdeen had been making for rigs in the Jasmine Field
when a low oil pressure warning was indicated.

A woman passenger was rescued from Belfast Lough yesterday evening
after falling from the Stena Super Fast VII ferry as it approached
Belfast Harbour. The woman was admitted to the Royal Victoria Hospital
in hypothermic condition. It was also believed that a 9-day old baby
had fallen into the water and was missing, Bangor Lifeboat was
attending the incident.

Record fine over piggery accident
A record fine of £187,500 plus £13K in costs was levied against JMW
Farms at Belfast Crown Court in respect of a fatal accident at its pig
farm in Tynan, County Armagh, on 15th November 2010 when a 45-year old
worker died when a 200kg bin fell from a forklift truck at a meal
mixing plant. He had been cleaning the bin from an adjacent position
on a bale compactor, and as the bin was moved he jumped on to the side
of it, but instability caused the bin and worker to fall to the
ground. Investigation found that the bin was inadequately attached,
with the forks not fitting the bin's sleeves.
The forklift was replacing another machine
undergoing servicing.

Downer EDI Works was fined $187,500(£117,366) over an accident in May
2009 in which a worker was severely injured in a fall into a pit
during cleaning work on a machine used in the preparation of asphalt.
He became crushed between an incline conveyor belt and a drum roller,
sustaining fractures and injuries which left him a partial
tetraplegic. Investigation found that there was a lack of guard
railing around the machinery.

Fire destroys Falmouth hotel
Fire broke out on the 3rd floor of the 4-storey Falmouth Beach Hotel
yesterday, completely destroying the 120-bed hotel in Gyllyngvase,
Falmouth. Best Western has been operating the hotel for the last 12
years, no injuries were reported and it appeared that all staff and
guests have been accounted for.
