23-year old worker died and another was injured.

Texas meat worker's arm severely injured
A worker was admitted to the Baylor Medical Centre in Dallas with a
severe arm injury following an accident at Frontier Meats on North
Grove Street, Fort Worth. It was initially speculated that he may have
had an arm amputated in the accident but this remained unclear.

A 60-year old worker was killed in an accident involving machinery
which grinds limestone with a vacuum at US Lime plant in Marble City,
Oklahoma. Initial investigation speculated that the machine was
activated accidentally, drawing the worker in, fire crews took 3 hours

A Bosnian international youth footballer has drowned in an accident in
an open air swimming pool in Ilvesheim where he was participating in
an international U-19 football tournament, life guards were unable to
resuscitate him after his body was found in the bottom of the pool.

Worker bitten by Komodo dragon
A worker was bitten by a Komodo dragon, the world's largest lizard,
while working in a timber house in on Rinca island. He was bitten on
the hand by the dragon, known as 'oras' to the locals, but managed to
beat its snout and secure his release.
The lizards can exceed 90kg in weight but can be dissuaded from attack
by adopting an aggressive posture. However, their saliva carries 50
strains of virulent bacteria and the worker's condition after the bite
was unknown.

19 killed in Jiangxi derailment
A train travelling from Shanghai to Guilin derailed in the mountainous
area of Dongxiang, Jiangxi province, on Sunday, where it struck a
landslide following locally heavy rains.
The locomotive and a number of the 17 carriages derailed, killing at
least 19 people and injuring another 71.

Gironde parachutist killed
On Sunday an experienced 28-year old parachutist died when his
parachute tore during a manoeuvre with 3 other parachutists at
Floudes, in the Gironde. He had jumped at 4K metres and his
parachute had opened normally at 1K metres, but it tore as he
performed the manoeuvre at 200 metres without time to activate his
reserve parachute.

Worker killed in unstable excavation in Joburg
A construction worker died in an unstable excavation on Thursday in
Linden, north Johannesburg, during the work in preparation for the
laying of sewer lines.
Two men were in the trench when it collapsed, the victim was
completely buried, his colleague managed to pull himself out although
soil was up to his chest.

An explosion at a Turkish Anthracite Coal Foundation mine in Karadon,
Zonguldak, left 32 miners trapped and 11 injured on Monday.
Initial speculation suggested firedamp, or a possible electrical

after the explosion. Rescue teams were using an adjacent shaft to

Wiener Lienen has announced that all 4,400 doors of its fleet of

in the wake of an accident in which a 5-year old boy's foot jammed in

His leg was released when he crashed into boarding further along the
platform and his condition is stable after treatment for a broken

At Lincoln Crown Court Steel Construction Ltd of Wyken, Coventry, has
been fined £50K plus £22K costs after pleading guilty to breaches of
health and safety legislation in respect of an accident on February
8th 2007 at a construction site in Glossop where a worker was left with
debilitating injuries in a 9-metre fall while installing a new racking
system. SDI Group UK Ltd of Cambridge was also fined £80K plus £20K in
costs; and the managing director of RM Berwick Steel Erection Services
Ltd of Isham was banned from directorship for 4 years and fined £40K

Methane explosions kill 12 miners, leave
83 trapped
Explosions in the Raspadskaya coal mine in Mezhdurechensk, Kemerovo,
have left at least 12 people dead and 83(64 miners and 19 rescuers)
trapped. Around 360 miners were underground in the 500-metre deep mine
when the first explosion, probably methane, occurred; a secondary
explosion destroyed the air supply system and much of the above-ground
structure and left rescuers trapped with the miners. Smoke-logging and
the poor quality air saw the rescue effort temporarily suspended on

Staten Island ferry rams pier

injuries when the Andrew J Barberi ferry sailing from Manhattan to
Staten Island rammed the pier at the St George Terminal.

Confined space accident in ship's hold
Two crew members, the chief officer and a seaman, have died in a
confined space accident in the hold of the Trans Pacific Containers
ship, Wellington, which was berthed at Marsden Point, Northland. The
Korean chief officer of the Panamanian-flagged ship had entered the
hold, possibly to check the timber cargo, believing that the fumigant
aluminium phosphide had not been added. The Burmese seaman and a
Korean 1st engineer attempted his rescue, the former died, the latter
is recovering in hospital. Other crew members later used breathing
apparatus to access the hold.

Family settled over mental distress claim
A Polish woman has won an €85K(£73,913)
settlement after taking a case to court alleging negligence and mental
distress in respect of an accident 5 years ago in which her son died
when a crane near to where he was working as a sling man contacted
overhead power lines.
She took her action against his employer Booth Precast Products Ltd, J
& M Healy Construction who owned the crane and JRD Developments
claimed to be responsible for the management of the site.
All parties had denied failing to provide a safe workplace and failing
to ensure the crane operated in a manner without contacting the
overhead lines.

Falling pipe killed worker during hoisting
At Ontario Court of Justice, Ottawa, S & R Mechanical has been fined
$125K(£84,060) after pleading guilty to a breach of Ontario regulation
213/91 section 172(c) of the Occupational Health & Safety Act in
respect of an accident in which a worker died after being struck by a
pipe during a hoisting operation.
On September 8th 2008, the worker was guiding a 181-kg pipe into an
overhead rack when a hoisting anchor snapped, Ministry of Labour
investigation found that the anchor system was not properly installed.
The company pleaded guilty to failing as an employer to ensure that
the hoist was arranged to prevent the pipe from slipping or falling. 19th May 2010.

in the Salang Pass on a flight from Kundus to Kabul on Monday with 44

A senior politician of the Bosnian Podrinje Canton of Gorazde was

injuries consistent with striking his head against the windshield if

Company fined over fire safety breaches
At Chester Crown Court the owner of P and S Ashley Timberworks Ltd of
Sandbach has been fined £80K after pleading guilty to 8 breaches of
the Regulatory Reform (Fire Safety) Order 2005. Lack of a suitable and
sufficient risk assessment attracted a £30K fine; £20K for a failure
to take suitable and sufficient preventative and protective measures;
and £30K for 6 other infractions, including insufficient means of
raising the alarm in the event of fire and unsuitable means of escape.
Additionally, costs of £50,300 were to be paid to Chester Fire and
Rescue Service.
The company was taken to court as a result of repeatedly failing to
carry out essential fire safety work in the business.

Aircraft collides with car
It remains unclear whether mechanical failure or human error caused
the collision of an AT3 light aircraft and a car at North Weald
Airfield.
The pilot had aborted an attempted landing but crashed into the car
during a second attempted landing. The car's occupants were unhurt and
managed to pull the pilot clear of the burning plane. The pilot was

colleagues positioned a ladder to bear his weight until fire crews
arrived with hydraulic scissors to cut him free.

Strip in Waterbury where the 51-year old victim's arms were caught and

RWE Power fined over air compressor accident
At Cardiff Crown Court RWE Power was fined £8K plus £14K costs over an
accident at the Aberthaw Power Station in the Vale of Glamorgan where
a contract worker injured his arm working with a compressed air
system.
On 22nd February 2007, he opened the air compressor system which he
believed to be isolated but which was charged, the company pleaded
guilty to a breach of S.3(1) of the Health & Safety at Work etc Act

Worker survived crushing accident at Forest Gate site
KP Gardner Builders were fined £7K plus £6,969 in costs at City of
London Magistrates' Court after pleading guilty to a breach of S.3(2)
of the Health & Safety at Work etc Act 1974 over an accident at a site
in Field Road, Newham, where a 41-year old builder broke his back and
ribs when the bricks of an unsecured wall fell 7 metres down on him in
windy conditions at a partially constructed block of flats. HSE
investigation established there was a failure to properly plan, manage
and monitor work at the site.

injury after he slipped and fell on a chain saw with which he was

operator Nexus.

Garage ignored prohibition notice
The Garage Bow Street Ltd, of Rugeley, Staffs, now no longer
trading, has pleaded guilty to breaching S.33(1)(g) of the
Health and Safety at Work etc and has been given a conditional
discharge for 6 months but must pay £500 towards costs.
The prosecution arose after it ignored a prohibition order requiring
it to cease using a dangerous gas heater because of its potential to
release carbon monoxide gas. The notice also specified that the
exhaust flue was directed inside the building, the electrical wiring
system was unsuitable and the system required servicing by a competent
gas engineer.

Workers killed by unstable loads
An electrician was killed on Wednesday at a building site in
Feldkirchen when he was struck by falling material positioned by a
crane.
On Tuesday a worker died in a crush accident involving a heavy granite
section being unloaded from a vehicle in Kolsass, 10 miles east of
Innsbruck. Two workers were inside a container with the granite when
it became unstable, one of them managed to jump clear.

Health club and lift company fined
At Southwark Crown Court, London, Holmes Place Health Clubs and
ThyssenKrup were each fined £233K after pleading guilty to breaches of
health and safety legislation in respect of an accident in March 2003
in which a 32-year old woman died in a malfunctioning lift at
Broadgate Health Club, London. She was dragged several feet between
the lift and shaft wall when it dropped and stopped between floors.
The lift had malfunctioned over a number of months, the judge was
critical of a "culture of complacency." 17th May 2010.

Crush fatality at Niagara Falls paper plant
A 40-year old worker was crushed to death in an accident yesterday on
a production line at Normapac Industries in Niagara Falls. He was
capping the end of a 3-tonnne paper roll when a second roll slid down
and he became pinned between the roll and a wall. Initial
investigation speculated that he may have been in an area workers
should not access when machinery was activated.

Narrative verdict over disabled patient's accident and death
A Dublin Coroner's Court has delivered a narrative verdict over the
death of a physically and mentally handicapped patient who died from
complications after a bathing scalding accident.
On August 16th 2008, the 34-year old female patient was being bathed at
home by a care assistant of the Irish Wheelchair Association when she
sustained 10% burns to her feet and her lower legs. A week later in
hospital she died of aspiration pneumonia after cardio-respiratory
arrest.
The coroner observed that "the burns started a chain of events which
led to death due to aspiratory pneumonia." 12th May 2010.

Former employees found guilty of neglect in CO accident
At Tokyo District Court a 72-year old former president and 60-year old
former chief quality control officer of Paloma Industries have been
found guilty of professional negligence resulting in death and injury
in respect of a carbon monoxide poisoning accident involving a
defective gas water heater in 2005 in which a student died and
another was injured.
The ex-president received an 18-month jail term suspended for 3 years,
the ex-quality control officer a 12-month jail term suspended for 3
years. The court heard that both could have foreseen the fatal
accident and were accused of neglecting to perform their duties to
implement safety measures, such as product checks and recalls. 12th May 2010.

HSE has been informed of a transport accident involving a lorry in which a worker
sustained chest
injuries from which he later died at a building site on North
Road, Aspatria, a 27-year old worker was arrested in connection with

A municipal maintenance worker died in a confined space accident in a
10-metre deep sewer manhole in Yankee Road, Middletown, Ohio. Three
firemen believed they were responding to a fall from height incident
and approached the locus without breathing apparatus, all required
hospital treatment, 1 having become unconscious in the
oxygen-deficient atmosphere, estimated at less than 2%. Initial
testing pointed to a nitrogen-rich atmosphere. 10th May 2010.

accident; and one died in a rock fall in a ledging panel 1100 metres

A preliminary report on the fatal accident in Oslo in March in which 3
workers died when 16 runaway freight wagons struck a harbour terminal

A 20-year old student participating in an outdoors event as part of

Flue pipe was dangerous
J S Loft Conversions Limited of
Lydney has been fined £2,000 with costs of £1,103 for breaching
the Gas Safety (Installation and Use) Regulations 1998 after its work
at a residential property in Stroud left its occupants in danger of
carbon monoxide poisoning.
In March 2009, as part of a loft conversion project a chimney serving
a gas fire was removed and replaced with a flexible gas flue pipe. The
occupants asked a registered gas engineer to check the work and upon
inspection the engineer found the installation was 'immediately
dangerous' and reported it to HSE.
The company's subcontractor also pleaded guilty to breaching regs.8(1)
and 8 (2) of the same Regulations. and he was fined £750 with costs of
£250. 28th May

Worker was killed by bull
A fatal accident inquiry into the death of a self-employed farm worker
has determined that he was most likely crushed by a Holstein bull
after he entered a field where it and other cattle were loose.
The incident occurred last December on Cuil Farm near Castle Douglas.
It was unclear why he had entered the field on foot and placed himself
in danger but it was speculated that he had perhaps gone to retrieve a
tractor part found to be missing.

Fan removed worker's fingers
A worker at Roydon Granulation Ltd, plastics recyclers of Rochdale,
suffered a severe hand injury whilst attempting to repair the 1,450rpm
fan on a drying unit.
The company was prosecuted and fined £15,000 with £4,347 costs. An HSE
official commented: "By law, the preferred solution would have been
for the workers to switch off and lock off the power supply to the fan
with padlocks. If this was not possible, then temporary guards should
have been put in place. These or other equally effective
measures were not taken."

Worker fell through void in
floor
Fernwave Limited was fined £30,000
plus £1,700 costs at Laganside Crown Court after breaching safety
legislation in connection with the death of a worker at the Obel Tower
site in Belfast on 11th December 2008.
The industrial accident occurred during the removal of temporary
supporting structures under the 4th floor where a plywood board
beneath an empty service void was left unsupported. The area around
the void was not signed or fenced off and when the plywood gave way
the deceased fell around 10 metres, landing on the concrete surface of
the first floor.

Council fined over vehicle
accident
Bridgend County Borough Council
has been fined for an incident in which a driver broke his leg after
his dumper truck overturned in woodland near Moel Gilau Lane, Bettws,
on 25th September 2008.
The truck began to slide on a gradient and then toppled, crushing the
driver's leg between the vehicle and a tree.
The Council was fined £10,000 plus £5,623, HSE found that there was no
evidence that staff were adequately trained in the use of small items
of plant equipment such as the dumper.

Manager crushed to death
On 6th April 2006, an admin manager employed by Roadways Container
Logistics Limited, of Stourton, Yorkshire, 2 cargo handlers and
a supervisor were unloading cases of glass from an open top freight
container at its premises.
As the final case, weighing approximately 2 tonnes, was being lifted
from the container, the manager was crushed when he tried to stop it
falling.
Investigation found that none of those involved had received the
appropriate training in lifting operations, no risk assessment or
formal planning was carried out prior to the incident and the case had
not been properly secured.
Roadways admitted breaching S.2(1) of the Health and Safety at Work
etc Act 1974 at Leeds Crown Court and was fined £250,000 with £100,000

SITA UK fined for site death
A driver died at SITA UK's landfill site in Cranford, Northants, in
January 2007 when he was crushed between his vehicle and a bulldozer.
He arrived at the site to empty his refuse lorry but had to be
assisted onto and off the tipping area because of poor ground
conditions. After being towed to the tipping area by a bulldozer, he
discharged part of his load, and to shed the rest he had to move
forward, but his lorry became bogged down. The driver of a compactor
radioed the bulldozer driver to say he would push the refuse vehicle
forward with his own vehicle. At the same time the bulldozer reversed
up to the front of the refuse vehicle to give a tow, but when the
compactor started to push the lorry forward, its driver was attaching
a tow rope from the bulldozer to the front of his lorry and he was
crushed between it and the bulldozer.
These working arrangements had been introduced before the accident
without having been properly risk assessed, and SITA had not defined
the supervisory roles.
SITA, of Maidenhead, Berkshire, admitted breaching S.3(1) of the
Health and Safety at Work etc Act 1974 and was fined £210,000 with
costs of £38,000.

Farm animals pose risk to public
The death of 78-year old man walking his dog in a field in
Middleton-by-Youlgreave in August 2008 illustrated how dangerous farm
animals can be to anyone entering their field, particularly dog
walkers.
The man was trampled to death by the cattle, probably after being
disturbed by his dog.
An inquest jury in Chesterfield determined an accidental death
verdict. 20th May 2010.

Building worker survived 7-metre fall
A building worker who fell through a factory roof light, hit a gantry
crane and landed on machinery below, he survived but suffered
fractured vertebrae.
He fell through the rooflight at the industrial premises in Caxton
Hall, Hertfordshire,

His employer, Hartog Hutton Ltd of
Bury St Edmunds, was fined £10,000 plus £7,076 costs.
Fluorocarbon Company Ltd, of Caxton
Hill, Hertford, which had contracted Hartog Hutton to carry out the
repairs to its factory roof, admitted breaching safety law and
was fined £5,000 with £5,195 costs.

Raleigh fined £72,000
A forklift truck driver died when he was struck by a lintel and debris
dislodged by the mast of the lift truck he was operating at Raleigh's
premises in Eastwood, Nottingham, in September 2007.
Investigation found that a lift truck had struck another doorway some
years earlier and the company had heightened that entrance, but the
entrance involved in the fatal accident could not be similarly or
readily altered because of electrical equipment above it. Raleigh was
not prosecuted over the death but its failure to ensure a safe
workplace. 19th May 2010.

Hospital Trust prosecuted after drug
mix up
Great Western Hospitals NHS Trust
has been fined £75,000 plus £25,000 costs in connection with the death
of a 30-year-old patient on 11th May 2004 at Marlborough Road
Hospital.
The deceased patient had just given birth
and was wrongly given a rarely-used local anaesthetic, bupivacaine.
She had been prescribed a saline solution to help raise her blood
pressure, but instead the bupivacaine was selected due to a mix-up.
An HSE official explained: "Had the
hospital done something as simple as keeping these completely
different but almost identical-looking drugs in separate cupboards,
then the patient would not have died. It is really important that
risks are properly assessed and safe systems put in place that
minimise the chance of human error."

Fall left worker with disability
Back injuries sustained by a worker who fell 3 metres from a retaining
wall in Burley-in Wharfedale during November 2008 have left him with
long-term disablement. An employee of construction company Kendal
Varley Ltd, he worked without the benefit of a safe system of work at
the time of the accident. The company was fined £2,000 with costs of
£1,800 having failed to meet duties under the CDM Regulations.
Principal Contractor Illson Ltd of Burley was fined £5,000 with costs
of £1,800 for breaching the CDM Regulations and the Work at Height

Building collapse worker broke
leg
Workers at an office block under construction in Kirkham, Lancs, were
deemed lucky to have avoided more serious consequences after it
collapsed because a concrete block pillar, used to support the first
and second floors, had been resting on the ground floor instead of
going down into the foundations. One of the workers broke his leg in
the collapse.
The building designer, Wallace Partnership, of Preston, admitted
breaching S.3(2) of the Health and Safety at Work etc Act 1974 for
which it was fined £4,000 with £12,318 costs. Principal contractor
Jack Smith (Builders) Ltd, of Preston, admitted breaching S.3(1) of
the same Act for which it was fined £3,000 with costs of £12,318.

College failed window cleaner
A
college has been prosecuted because it
failed to perform its legal duty to check that its contractors had
proper procedures in place.
A window cleaner, employed by A Nicoll & Son Ltd, was contracted to clean windows at Lincoln College
on 4th November 2008. As he accessed the roof of the main reception
building using a ladder, he leaned against the building to clean
nearby windows and fell, sustaining injury that prevented him working
for months.
Lincoln College admitted breaching r.3(1) of the Management of Health
and Safety Regulations 1999, having failed to conduct a suitable and
sufficient risk assessment. It was fined £1,500 plus £9,500 costs.

An electrician sustained serious injury when he fell from a ladder
while working between decks of the vessel, Our Julia, when it was moored
in Scarborough harbour on 16th July last year.
Fishing boat operator Our Julia Ltd, of Cayton, admitted
breaching r.8(e) of the Work at Height Regulations 2005 and was fined
£7,000 with £2,028 costs.
The accident victim will not be able to work as an electrician
following his injuries which were described as horrific by an HSE

Catastrophic event avoided but Calor Gas
fined
Calor Gas Ltd has been fined
£27,500 plus £22,185 costs for permitting a major LPG gas leak at its
Canvey Island terminal in Essex during October 2008.
While LPG was being unloaded from a ship, a leak occurred when an
over-pressure safety device operated, releasing 163 tonnes. A sensor
to detect such leaks failed to operate and an employee discovered the
leak, but no site emergency alarm was sounded.
Calor Gas Ltd admitted breaching r.4 of the Control of Major Accident
Hazards (COMAH) Regulations 1999 and r.3(1)(e) of the Reporting of
Injuries, Diseases and Dangerous Occurrences Regulations 1995.
HSE's Head of Chemical Industries, Eastern Region, commented: "As was
said in court, this could have had the potential of becoming a
catastrophic event." 11th May 2010.

Storage failure at joiners
A man damaged his knee ligaments when a number of MDF boards crushed
him against a machine. He was assisting a colleague to retrieve a
large piece of MDF from the middle of a vertical storage stack when he
collapsed under the stack’s weight.
At Harlow Magistrates' Court his
employer, Corvale Ltd of Harlow, was fined £2,667 plus £2,460 costs, it
had failed to provide a safe system of storage and management of MDF
and other sheets.

An as yet unidentified person died in last week's fire at the
Majestic Hotel in Harrogate. There has been speculation that it may
be a member of staff since unaccounted for.
More than 100 guests and
25 staff escaped the fire which broke out in staff accommodation on

Steel smashed teenager's leg
A 17-year old employee of Dranson Ltd of Wolverhampton was pushing 0.7
tonnes of steel on a trolley over a ramp when a wheel became wedged in
its lip and toppled it to one side, falling onto him, pinning him to the
ground and breaking his right leg in several places.
The trolley was
not suitable for outdoor use and the floor surface had not been
maintained.
Dranson was fined £8,000 plus £3,603 costs.

C.M.A Tools (Burnley) Ltd has been fined £1,000 plus £1,000 costs for
breaching health and safety law in the circumstances of an industrial
accident in which an employee sustained a serious injury to his hand.
The worker was applying a piece of emery cloth to a metal component on
a lathe when it became trapped by the rotating mechanism. He was
wearing a glove at the time and this pulled his hand further into the
machinery. The accident victim, who lost a finger, had not received
training on how to operate the lathe safely, or on the dangers of
using emery cloths on metalworking lathes.

Dreamspace deaths accidental
An inquest jury has determined accidental death after listening to the
facts surrounding the July 2006 accident at Riverside Park in
Chester-le-Street, County Durham, in which an inflatable artwork
escaped its anchors with 20 visitors inside it.
Expert opinion indicated that 108 anchors were required to stabilise
the structure but only 22 were discovered.
Two women aged 38 and 68 were fatally injured when they fell from it.

Fairground incident injures man
It is reported that a man was thrown from a fairground ride on Monday
afternoon at the Little Roodee car park in Chester. He was taken to
hospital where he was treated for non-life-threatening injuries.

Steel load slipped and struck worker
A 58-year old worker employed by John Doyle Construction Ltd suffered
what were described as 'terrible' injuries when a load of steel being
slung slipped when the chains came together on a residential site in
Victoria, London. The 2.7-metre beams being slung hit a piece of
reinforcing bar protruding from a first floor slab, causing it to
become unstable and injure the worker to the extent that he has been
unable to work since. Investigation found the lift was not carried out
in a safe manner, the site was congested with building materials, and
HSE maintained that there should have been a specified safe zone for
lifting.
The company of Welwyn Garden City, Hertfordshire, admitted
breaching r.8(1)(c) of the Lifting Operations and Lifting Equipment
Regulations for which it was fined £2,000 with costs of £17,466.

Bread maker fined
A dough mixing machine accident resulted in an employee having a
finger amputated. The man was using a spiral mixing machine to make
dough in October 2006 at the Bread Factory Ltd's premises in Hendon,
his left hand came into contact with the unguarded beater of the
mixing machine. The guard had been missing for some time, the company
pleaded guilty to breaching r.11(1) of the Provision and Use of Work
Equipment Regulations 1998 and was fined £3,500, plus costs of £2,926,
at the City of London Magistrates' Court.

14-year old killed in training ship
fall
A boy, 14, has died after being recovered from the sea by a rib from
his sail training ship TS Royalist. The incident occurred at Stokes
Bay, Hampshire, on Sunday evening as he climbed the rigging, the Marine
Accident Investigation Branch and Hampshire police are investigating.
