Poland 8 Silesian miners killed in minibus crash Eight Silesian coal miners died when their minibus was involved in a collision with a timber-laden lorry between Zywiec and Wegierska Gorka, 45 miles south of Katowice. The miners were returning from a shift at the Myslowice-Wesola colliery. Ten others survived the accident but some are listed in life-threatening condition. 30th March 2012 Canada Ottawa forklift accidents The Ministry of Labour is investigating 2 forklift accidents in Ottawa; at premises on Colonnade Road a worker's leg was broken when he was struck by a forklift; at a site on Besserer Street metal fell from a forklift and inflicted leg injuries to a 21-year old worker. 30th March 2012 Germany Machine rolls down ramp and crushes worker after cable snaps A 56-year old worker died on Wednesday morning after being hospitalised overnight with crush injuries following an accident at the Holzheim, Hessen, open air swimming pool. He was working with a contract team relaying the pool floor. Machinery was being lowered down a ramp by means of a steel cable attached to a vehicle. He was transferred to hospital in Hamilton with serious injuries, the Ministry of Labour is investigating the accident. 23rd March 2012 China 7 killed in accidents at Henan highway sites Seven workers have died in separate accidents this week on 2 sites where construction of the new Luoyang-Luanchuan highway is being undertaken in Henan. At one site 5 died when a scaffolding fell in activity involving an allegedly unlicensed crane operator; at another site 2 workers died in a fall from a bridge. 22nd March 2012 Canada Jaws of Life rescue in New Brunswick Jaws of Life were required to rescue a crane operator who was pinned against his control system by a train rail which became unstable when a mechanical failure occurred during a lifting operation at a site in New Brunswick. Although the operator was seriously injured he is not listed in life-threatening condition.  22nd March 2012 Turkey Roof collapse and CO2 kill miners in separate incidents Three miners have died in separate accidents. He was opening a ventilation system valve between levels 7 and 8 when the accident occurred.  16th March 2012 France Two sugar silo cleaners buried under slippage Two contract workers of Carrard Services of Reims died in a crush accident in a silo at the Cristal Union sugar plant in Bazancourt, Marne, where they were buried under 3K tonnes of sugar. 15th March 2012 Bangladesh Dhaka female construction worker killed in wall collapse A wall collapsed on a construction site on the Glory Real Estate, Shekhertek, in the Adabar area of Dhaka, killing a female construction worker and leaving a male colleague injured. 15th March 2012 Bangladesh Scores missing in ferry sinking At least 32 people drowned when the MV Shariatpur-1 ferry collided with a cargo boat on the River Meghna 20 miles south of Dhaka. Divers entered the sunken boat in 25 metres of water to recover 31 bodies but scores of others are feared missing. On Monday a 38-year old worker severed his hand while cutting metal at premises in Asker, his condition was first listed as critical but now stable in Salmaniya Medical Complex. 14th March 2012 Spain Fatal accident at Albacete airfield The Ministry of Defence is investigating a fatal accident which occurred yesterday at Maestranza Air in Albacete where a worker was killed in an accident involving a tractor being used to service aircraft. 13th March 2012 Belgium Antwerp barbecue fire leaves worker critically burned A 29-year old man is listed in life-threatening condition in Stuivenberg Hospital, Antwerp, with burns to his face, arms and upper body after being burned by shooting flames from a barbecue in a park on Marcel Auburtinlaan in the Berchem area of the city. A 27-year old colleague has severe burns on his hands and arms but is not in life-threatening condition. It is believed that bio-ethanol was being introduced as fuel when the accident occurred. 13th March 2012 Turkey 14 workers killed in fire at Istanbul site Fourteen construction workers died when fire broke out in their tented dormitory accommodation at the site of a new shopping mall in the Esenyurt district of Istanbul. It is speculated that an electrical short circuit occurred in a cooker and the flames spread to 3 tents at the site. 12th March 2012 Argentina Drilling accident at Andean site A fatal accident occurred yesterday at the Del Carmen project 50 miles south of the Veladero mine in the Iglesia department of San Juan where a miner of DAG XLSRL died in an accident while operating drilling machinery. 12th March 2012 Austria Workers burned to death in aluminium plant oven Two workers were burned to death in a preheating oven on Thursday at the Salzburger Aluminium plant in Lend. USA Forklift fatality at Georgia Ports terminal A 57-year old female worker died after being struck by a forklift truck at Berth 13 of the Ocean Terminal, Savannah, where the Ports' America stevedore company was working on the Arietta berthed there. 30th March 2012 USA Texas arborist electrocuted A 37-year old arborist of Friendswood Firewood was electrocuted after contacting an overhead power line while trimming trees with a pole saw in Friendswood, Texas. 30th March 2012 Panama Fisherman survives 1 month adrift An 18-year old fisherman has survived a month adrift in the Pacific Ocean when engine problems beset a Rio Hato fishing boat, 2 colleagues died of dehydration and exhaustion. The survivor was picked up off Ecuador's Galapagos Island by the fishing boat Duarte V, having survived on raw fish and rainwater.  29th March 2012 Ecuador Mudslide buries Peruvian miner A 21-year old Peruvian miner died in a mudslide in El Trebol coal mine in the Bella Rica area in Camilo Ponce Enriquez, south Ecuador, arrangements are underway to repatriate his body from Azuay province to Peru. 29th March 2012 Colombia 2 killed, 1 injured in separate mine accidents Two Cundimarca miners have died and another was seriously injured in separate incidents. At the Aliso mine in Penas, Guacheta, the pulley of a  drilling rig collapsed and killed a miner. At the Cerromatoso mine in Santuario, Guacheta, a 41 year old worker was killed in an accident during the unloading of coal from a wagon. And a roof collapse trapped a miner at La Ramada mine in Ramada Florez, Lenguazaque, the extent of his injuries was unknown. 28th March 2012 Costa Rica Rider killed during bull riding competition A 21-year old man died from injuries sustained in a bull riding competition during a local festival organised by Asociacion de Desarrollo Integral in El Futuro de la Tigra in San Carlos, Alajuela. He was twice thrown against the barrier of the arena by a 750Kg bull he was riding and died later in Hospital San Carlos.  27th March 2012 Venezuela Worker falls 6 storeys at Libertador site A 33-year old construction worker, thought to have been an employee of Adonais Square Construction, died in a fall from 6th storey level at the Los Frutales newbuild site in  Libertador, Caracas. 27th March 2012 USA Unstable machinery kills Utah factory worker Machinery which moulds rubber sections in the manufacture of conveyor belts became unstable and fell and pinned a 34-year old worker who was killed on Friday at Cambelt International Corporation's plant in Salt Lake City. 24th March 2012 USA Fatality at New York demolition site A 1-storey building collapsed yesterday at a demolition site on 130th Street, Manhattan, claiming the life of a worker and injuring 2 others. Breeze Construction of Brooklyn has been working there where a Columbia University building is being demolished as part of a new development project. It is speculated that a support beam had been cut just prior to the incident. 23rd March 2012 Austria Hotel sauna fire Five people were badly injured when fire broke out in the Felsen sauna at the Vital Hotel in Heilbrunn. It was speculated that a water mixture which was introduced contained a flammable element. One of the injured is listed in life-threatening condition with 70% burns, another has 30% burns. 22nd March 2012 USA South Carolina forklift fatality A fatal accident investigation is ongoing at premises in Fort Mill, South Carolina, where a worker was killed in an accident involving a forklift truck. 22nd March 2012 Spain Huelva worker killed in 15m roof fall A 21-year old subcontract worker of ENCE has fallen to his death from the roof of a building at the Celulosas factory in the San Juan del Puerto area of Huelva. Although the metal weighed a quarter of a tonne the 26-year old emerged from the accident with no fractures of the legs.16th March 2012 Germany Falls through a scaffold and roof A timber board failed on scaffolding during work on a site on Steinbruch Street, Pfrondorf, causing 2 workers to fall 3 metres, both were hospitalised in Tubingen with head and leg injuries. She is in intensive care in South Tipperary General Hospital, Clonmel. 14th March 2012 England Falling steel kills Cumbrian worker A steel section being hoisted by crane fell and killed a worker yesterday on the Meeting Industrial Estate, Barrow, a 31-year old colleague was admitted to Furness General Hospital with non life-threatening injuries. 13th March 2012 Singapore Slab being excavated kills worker at rail site A falling concrete slab being removed from deep within an excavation at an MRT(Mass Rapid Transport) Line construction site on Evans Road has killed an excavator operator working 12 metres down in the excavation. The Land Transport Authority is investigating the accident which saw a 10-tonne jack and airbags required to lift the slab, an operation which lasted 5 hours. 13th March 2012 Canada Alberta road sweeper killed in traffic accident near oilfield site A 33-year old operator of a bobcat road sweeper, an employee of GMack Oilfield Services, died in a transport accident at the weekend when his vehicle was in collision with a lorry as he was cleaning the road surface near the site 125 miles northeast of Edmonton. 12th March 2012 USA Michigan road surveyor killed in Detroit work zone On Saturday morning a contract surveyor of the Michigan Department of Transportation was killed in an accident when he was struck by a vehicle in a work zone on Interstate 94 in St Clair Shores, Detroit. 11th March 2012 Italy Two workers killed at Piedmont roads site A civil engineer and a construction worker were killed on Thursday when they were struck by a timber-laden lorry at a work site near the Ivrea junction on the Turin-Aosta motorway, initial speculation was that the driver may have fallen asleep at the wheel.  9th March 2012 Ecuador Worker killed in trench collapse A trench collapse has claimed the life of a construction worker on Paseo Tres de Noviembre in the Las Escalinatas district of Cuenca where work was ongoing to install a water line. The trench was 6 metres deep and it is thought that an inability to access an area of it with an excavator saw the worker entering to hand dig. 9th March 2012 Germany Chain snaps during loading work in Rostock A 34-year old worker was hospitalised in Rostock on Wednesday with a severe hip injury after being struck by falling steel plate when a chain failed as the material was being loaded on to a lorry in Polchow. He was standing on the lorry when he was struck. 8th March 2012 Canada Worker dies after leg artery cut A steam pipe fractured under pressure at a site at Christina Lake, 40 miles south of Fort McMurray, and a 62-year old worker was struck so violently on the leg that a fatal blood loss occurred as a main artery was cut. He was transferred to Lac La Biche Hospital in critical condition but died shortly after admission. 8th March 2012 USA Race track worker killed in bolting horse incident A bolting horse accident led to the death of a 24-year old track worker at the Remmington Park race track in Oklahoma City. He was positioned 300 metres behind the starting gate when the horse bolted. In trying to prevent the animal leaving the track he was struck by a gate through which the horse crashed. 7th March 2012 USA Iowa arborist electrocuted An Iowa arborist has been electrocuted at a site 2 miles south of Nemaha where he was trimming a tree. He was leaning against a MEWP(mobile elevating work platform) which contacted an overhead power line, a colleague in the raised bucket was uninjured. 7th March 2012 Luxembourg Worker trapped in Esch manhole A Sanem worker was hospitalised with unspecified injuries after being rescued from a manhole into which he fell yesterday during sewer maintenance work on the Esch highway between Belvaux and Esch-sur-Arzette. Fire crews reported that earth ingress occurred and he was trapped by the legs at the bottom of the manhole. 6th March 2012 USA Exploding wheel kills Alabama tyre fitter An Alabama tyre fitter was killed in an accident when an HGV tyre exploded during installation at TCI Commercial Centre in Decatur. England Tyne and Wear industrial oven fire Fire broke out in an industrial oven at a business unit on the Bedewell Industrial Estate, Hebburn. A worker sustained burns to his lower arms but declined further hospital treatment after being attended to by a paramedic.  A pallet burned out in the oven and there was a degree of smoke logging in the unit. 30th March 2012 France Dutch aircraft diverts after pilot becomes ill An ArkeFly aircraft flying to Schiphol from Banjul in the Gambia had to make an emergency landing in Bordeaux last night when the pilot became unwell. 29th March 2012 Tunisia 4.5% decline in work accidents Workplace accidents for 2011 fell to 43K from a figure of 45K in 2010, public works construction involved the highest figure(11.38%) with most accidents(17.79%) being recorded the Sfax area. 29th March 2012 Wales Boy's death in goal frame accident prompts FAW to issue safety guidelines In response to the death of a 12-year old Powys boy who sustained fatal crush injuries and liver trauma when a goal frame fell on him during play on a recreational park in Caersws last July, the Football Association of Wales has issued new guidelines for the safe and correct use of goal frames at all time during matches and training. He was wearing flip flops and had no life jacket or personal locator beacon. In a state of shock the skipper had failed to give the coastguard the correct information of the boat location and failed to perform an emergency drill upon realising there was a man overboard. The boat's risk assessment had also failed to consider hazards posed by gear stored on deck and the victim had failed to complete a mandatory Seafish safety awareness course. 14th March 2012 Poland Silesian miner killed A 26-year old miner died in an accident on Monday 900 metres underground in the Staszic mine, Katowice, where he and a colleague were engaged in the construction of a conveyor belt system, the other miner was hospitalised with leg injuries. 13th March 2012 Spain Excavator driver killed in overturn accident at Leon mine An excavator driver was killed at the weekend when his vehicle left the road and overturned into a 3-metre deep water-filled ditch at the Jarrinas mine at Santa Cruz del Sil, Leon.  12th March 2012 Rep Ireland Kerry farmer dies in crush accident A Kerry farmer was killed on Saturday in an accident on his farm in Listry where he became trapped under a tractor and trailer. He was transferred to Kerry General Hospital but succumbed to his injuries. Later in the day doctors managed to revive the worker. 8th March 2012 Canada Fireman drowned in ice rescue training exercise In court in Sarnia, Ontario, the Corporation of Village of Point Edward was fined $75K(£47,761) over a fatal accident in which a member of the volunteer fire department drowned in an ice water training exercise. On January 30th 2010, members of the fire department were swimming to moving sheet ice with a view to mounting the ice and floating down to the lake. Some were unable to climb up and swam back. One who was unable to climb ashore was pushed under the surface by an ice floe and was trapped underneath for 4 minutes. Ministry of Labour investigation established that although rescue equipment was there it was not readily available to pull him ashore. The Corporation pleaded guilty to breaching the Occupational Health & Safety Act Section 25(2)(h). 8th March 2012 Germany Bull kills Friesland farm worker A farm worker was killed by a breeding bull on a farm in Immenstadt, North Friesland, on Tuesday morning. England Wilts man dies under tractor wheels A 67-year old Wiltshire man died in a fall under the wheels of a moving tractor onto which he was attempting to climb to apply the brake after it began to move on a gradient during unloading from an attached trailer on Conkwell Grange Estate in Limpley Stoke. 30th March 2012 England Company fined over Work at Height breach At Runcorn Magistrates' Court Hutchinson Technical Services Ltd was fined £10K plus £3,588 in costs over an accident at its Widnes site where a worker was injured when he fell 2.5 metres on to a metal girder from the top of a tank inside which chemical vapour ignited and exploded. There were no fall prevention measures in place and the company pleaded guilty to breaching r.6(3) of the Work at Height Regulations 2005 by failing to ensure that suitable measures were in place to prevent a worker being injured in a fall. The company promptly rectified this by installing cages on top of tanks and introducing safety harnesses. 28th March 2012 India Wall collapse kills worker at refurbishment project A wall collapsed during demolition work as part of refurbishment of a National Highways building in Coimbatore, Tamil Nadu, killing a 35 year old worker buried under the debris. 27th March 2012 Austria Tyrol workplace accidents Two workers were hospitalised in Innsbruck in Monday after separate accidents. In Telfs a worker drilling at the Mittler Inn power plant crushed his hand in a pulley; in Rippenbruch a 61-year old farmer fell through the roof of his byre when he stood on a charred timber beam as he inspected fire damage on the roof. 27th March 2012 Austria Divers seek to recover bodies from lake Constance aircraft crash Divers resume their work today(Saturday) to reach the wreckage and recover the bodies of 2 people who died when their light aircraft crashed into Lake Constance less than half a mile off Wocherhafen, Bregenz. The sightseeing flight had departed from Hohenems airfield on Friday afternoon. The wreckage is lying in 40-50 metres of water, making the divers' task more demanding. 24th March 2012 England Met officers badly injured in dog attack Four Metropolitan Police officers  participating in Operation Big Wing to trace people wanted by the police or wanted in connection with non-court appearance have been admitted to Royal London Hospital after being attacked by a pit-bull dog while apprehending a 25-year old suspect at an address in Albert Square, Newham. Armed officers of the CO19 Specialist Firearms Command attended the incident and shot the dog which had inflicted serious bite injuries to 5 officers in total. 23rd March 2012 England Body lowered into wrong lair in Cumbrian cemetery Copeland Council in Cumbria has taken disciplinary action against a member of staff of Whitehaven Cemetery after a body was lowered into the wrong lair during a funeral there. The alert was raised by a passer-by who noticed a decorated Xmas tree visible in the property in which the resident is speculated to have lain dead for 3 months. 21st March 2012 Italy HGV overturns in gravel pit A lorry driver was killed yesterday when his vehicle overturned in a gravel pit at San Floriano in Egna, south of Bolzano. 20th March 2012 Sweden Recovery operation at Hercules crash site on Mt Kebnekaise Statens Haverikommission, the Swedish Accident Investigation Authority, has indicated that rescuers have recovered human remains near the site of a crashed Hercules military aircraft on Mount Kebnekaise. The risk of avalanche on Sweden's highest peak has posed risks for rescuers, it remains unclear why the C-130 J Super Hercules, manufactured by Lockheed Martin of America and carrying 5 personnel, lost altitude and crashed. It was participating in the Cold Response military training exercise. 20th March 2012 Italy Fatal accidents in Emilia Romagna and Abruzzo In Carpi, Emilia Romagna, the proprietor of a transport company died in an accident when he was struck by a lorry; at Civitella Messer Raimundo, Abruzzo, a 78-year old farmer was killed when he was struck by a 10-metre high oak tree being felled on his land. 19th March 2012 Germany Balcony collapse brings scaffold down Three workers aged 20,36 and 46 were hospitalised in Dachau at the weekend after a fall from height accident at a housing site in Olching, 10 miles northwest of Munich. The cause is under investigation, no injuries were reported. 16th March 2012 England Diver rescued from Channel shipping lane Dover Coastguard co-ordinated the rescue of a diver yesterday after the latter was spotted in the shipping lane by a ship passing the Sandettie South West Buoy in the Dover Straits. He was later safely returned to his dive boat. Dover Coastguard was unaware of his dive plans and stress the importance of advising the coastguard in advance of any plans to dive. 15th March 2012 Scotland Man dies in Loch Lomond caravan fire Central Scotland Police are investigating a fatal accident which occurred in the grounds of the Drovers Inn near Inverarnan where a man died in the early hours of yesterday in a fire in a caravan parked in the grounds. 14th March 2012 England Essex schoolgirl dies after being struck by rugby ball A 12-year old Essex schoolgirl died yesterday after becoming unwell during a PE lesson at Woodland Comprehensive School, Basildon. She was participating in a rugby lesson and she was attempting to catch a ball which struck her chest. She fell over and appeared to experience a fit. 14th March 2012 England Inadequately trained worker injured on defective machine At Lincoln Crown Court DS Smith Packaging of Louth was fined £50K plus £8,244 in costs over an accident in which a worker operating a machine which rolled up corrugated packaging sustained a multiple fracture of an arm and cracked ribs when he was thrown backwards as he operated it. Investigation established that he had been trained to use the machine by a worker who had not completed full training and the machine was defective and had a design fault. 13th March 2012 Scotland Body recovered from Fraserburgh harbour A body was recovered yesterday from the Balaclava Basin in Fraserburgh harbour, it was unclear whether the victim was a fish industry worker or member of the public. 12th March 2012 England West country hotel fined over toddler's fall At Bath Magistrates' Court The Old Mill Hotel was fined £10K plus £1,948 in costs over an accident in which a 2-year old girl who was a guest at a wedding fractured her skull in a 3-metre fall from a first floor window. A official of Bath and North East Somerset Council commented that safety measures in place at the window were inadequate. 9th March 2012 The Netherlands Generator in building leads to CO incident Seven people were admitted to the Erasmus hospital in Rotterdam yesterday for treatment for carbon monoxide poisoning after fumes built up in a building in the Delfshaven area of the city in which a generator was running. 9th March 2012 Scotland CO accident in tent A 49-year old camper was listed in critical condition in Borders General Hospital following a carbon monoxide accident while camping on Minch Moor. He was reported missing and was found 24 hours later in his tent in an oxygen-deficient atmosphere, the cause attributed to gas-powered cooking or lighting apparatus. 8th March 2012 Russia St Petersburg supermarket fire Fire led to a structural collapse on Wednesday morning at the K-Rauta supermarket on Marshall Govorovsky Avenue in the Kirov district of St Petersburg. The locus appears to have been the roof. It was unclear whether hot metal work was being undertaken, no news of injuries was reported. 8th March 2012 Wales Cardiff care home patient dies in lift shaft accident A 96-year old patient has died in a lift accident at the Pontcanna House care home in Canton, Cardiff, a staff member was also injured in the accident. 7th March 2012 Australia Unfooted ladder accident At Albany Magistrates' Court Ferngrove Vineyards was fined $60K(£40,260) after pleading guilty to failing to provide a safe workplace in respect of an accident in which a worker was injured in a 3-metre fall from an extension ladder while cleaning the outside of a 6-metre high wine vat. A WorkSafe inspector observed that the accident was avoidable if a colleague had assisted by footing the ladder as described in the company's health & safety manual. 7th March 2012 Norway Svalbard governor's verdict on British schoolboy's mauling by polar bear Following an accident in which a  British schoolboy was mauled to death by a polar bear last August while at a British Schools Exploring Society camp at the Von Postbreen glacier, 25 miles from Longyearbyen, the governor of Svalbard has indicated that no charges of criminal negligence are to be pursued. The bear breached a tripwire which failed to warn of its presence and a rifle failed to discharge adequately to repel the attack which also saw 4 others injured. The parents of the deceased Etonian scholar are expected to appeal the governor's verdict. 6th March 2012
