8 Silesian miners killed in minibus crash
Eight Silesian coal miners died when their minibus was involved in a
collision with a timber-laden lorry between Zywiec and Wegierska
Gorka, 45 miles south of Katowice. The miners were returning from a
shift at the Myslowice-Wesola colliery. Ten others survived the
accident but some are listed in life-threatening condition.

Machine rolls down ramp and crushes worker after cable snaps
A 56-year old worker died on Wednesday morning after being
hospitalised overnight with crush injuries following an accident at
the Holzheim, Hessen, open air swimming pool. He was working with a
contract team relaying the pool floor. Machinery was being lowered
down a ramp by means of a steel cable attached to a vehicle. When the
cable failed, the moving equipment pinned him against the pool wall.

A Stokes Basket rescue was undertaken yesterday at Seaway Marine &
Industrial at the Port Weller Dry Docks in St Catharines where a
53-year old worker fell 10 metres into the engine room of the vessel,
English River. He was transferred to hospital in Hamilton with serious
injuries, the Ministry of Labour is investigating the accident.

Seven workers have died in separate accidents this week on 2 sites
where construction of the new Luoyang-Luanchuan highway is being
undertaken in Henan. At one site 5 died when a scaffolding fell in

site 2 workers died in a fall from a bridge.

Jaws of Life were required to rescue a crane operator who was pinned
against his control system by a train rail which became unstable when
a mechanical failure occurred during a lifting operation at a site in

worker at the Remmington Park race track in Oklahoma City. He was

guidelines
In response to the death of a 12-year old Powys boy who sustained
fatal crush injuries and liver trauma when a goal frame fell on him
during play on a recreational park in Caersws last July, the Football
Association of Wales has issued new guidelines for the safe and
correct use of goal frames at all time during matches and training.
These include the dismantling and storage of portable goals when not
in use, with special focus on unsupervised playing fields.

OHS issued a stop work order at J & D Food Services in Edmonton where
a worker sustained a serious head injury, possibly a fractured skull,
when he became pinned against a loading bay wall by a reversing lorry.

Storage tank collapse at sand works
A 30-metre high cement storage tank collapsed on to a 2-storey
building and a parked vehicle at the Wangming sand works in Hangzhou,
Zhejiang. A woman and child have died in the accident which may have
also left another person trapped.

Safety failings in Pembrokeshire man overboard accident
The Marine Accident Investigation Board has concluded that the Latvian
fisherman lost overboard last June from the About Time fishing boat
which issued a mayday off Skomer Island most probably tripped or
slipped on ropes from pots stored on deck and fell un-witnessed over
the starboard bulwark.
He was wearing flip flops and had no life jacket or personal locator
beacon. In a state of shock the skipper had failed to give the
coastguard the correct information of the boat location and failed to
perform an emergency drill upon realising there was a man overboard.
The boat's risk assessment had also failed to consider hazards posed
by gear stored on deck and the victim had failed to complete a
mandatory Seafish safety awareness course.

Silesian miner killed
A 26-year old miner died in an accident on Monday 900 metres
underground in the Staszic mine, Katowice, where he and a colleague
were engaged in the construction of a conveyor belt system, the other
miner was hospitalised with leg injuries.

An excavator driver was killed at the weekend when his vehicle left
the road and overturned into a 3-metre deep water-filled ditch at the
Jarrinas mine at Santa Cruz del Sil, Leon.

Kerry farmer dies in crush accident
A Kerry farmer was killed on Saturday in an accident on his farm in
Listry where he became trapped under a tractor and trailer. He was
transferred to Kerry General Hospital but succumbed to his injuries.
It was not clear if he was changing a tyre at the time of the accident
which has been reported to the HSA.

Rail worker killed in fall from height
A CFF(Compagnie Ferroviaire Suisse) worker died in a fall from a work
platform while maintaining overhead lines on the track between Romont
and Fribourg yesterday, it appears that a passing goods train clipped
the wagon on which the platform was mounted, causing him to fall.

A sub-contract worker was admitted to Liverpool Hospital with several
fractures including all his toes broken when steel piping struck him
at Tahmoor Colliery, Oakdale, during unloading from a lorry, the load
became unstable as strapping was unfastened.

Worker pinned in press by hydraulic arm
A machine tool worker was admitted to hospital in Salzburg in a coma
after becoming trapped between a robotic arm and a hydraulic press at
a plant in St Pantaleon, Upper Austria. Later in the day doctors
managed to revive the worker.

Fireman drowned in ice rescue training exercise
In court in Sarnia, Ontario, the Corporation of Village of Point
Edward was fined $75K(£47,761) over a fatal accident in which a member
of the volunteer fire department drowned in an ice water training
exercise. On January 30th 2010, members of the fire department were
swimming to moving sheet ice with a view to mounting the ice and
floating down to the lake. Some were unable to climb up and swam back.
One who was unable to climb ashore was pushed under the surface by an
ice floe and was trapped underneath for 4 minutes.
Ministry of Labour investigation established that although rescue
equipment was there it was not readily available to pull him ashore.
The Corporation pleaded guilty to breaching the Occupational Health &
Safety Act Section 25(2)(h).

Bull kills Friesland farm worker
A farm worker was killed by a breeding bull on a farm in Immenstadt,
North Friesland, on Tuesday morning. The farmer found his worker dead
in the bull's enclosure, it is speculated that he had entered with
feed for the animal.

Asturian octogenarian killed by reversing site vehicle
An 80-year old man was knocked down and killed by a dumper vehicle
during maintenance and cleaning work in an area known as La Concha in
Porrua, Llanes. His son owns an adjacent agricultural business and the
elderly man generally crossed the area twice daily to attend to stock.

Missing Ukrainian fisherman's body found
A body was recovered yesterday in the nets of the fishing boat Rath
Elite 10 miles east of the Skerries, it is speculated to be that of a
Ukrainian fisherman lost overboard from the Zenith on 29th January
some 8 miles from Clogherhead, Louth.

Wilts man dies under tractor wheels
A 67-year old Wiltshire man died in a fall under the wheels of a
moving tractor onto which he was attempting to climb to apply the
brake after it began to move on a gradient during unloading from an
attached trailer on Conkwell Grange Estate in Limpley Stoke.

Company fined over Work at Height breach
At Runcorn Magistrates' Court Hutchinson Technical Services Ltd was
fined £10K plus £3,588 in costs over an accident at its Widnes site
where a worker was injured when he fell 2.5 metres on to a metal
girder from the top of a tank inside which chemical vapour ignited and
exploded. There were no fall prevention measures in place and the
company pleaded guilty to breaching r.6(3) of the Work at Height
Regulations 2005 by failing to ensure that suitable measures were in
place to prevent a worker being injured in a fall. The company
promptly rectified this by installing cages on top of tanks and
introducing safety harnesses.

A wall collapsed during demolition work as part of refurbishment of a
National Highways building in Coimbatore, Tamil Nadu, killing a 35
year old worker buried under the debris.

Two workers were hospitalised in Innsbruck in Monday after separate
accidents.
In Telfs a worker drilling at the Mittler Inn power plant crushed his
hand in a pulley; in Rippenbruch a 61-year old farmer fell through the
roof of his byre when he stood on a charred timber beam as he
inspected fire damage on the roof.

Divers seek to recover bodies from lake Constance aircraft crash
Divers resume their work today(Saturday) to reach the wreckage and
recover the bodies of 2 people who died when their light aircraft
crashed into Lake Constance less than half a mile off Wocherhafen,
Bregenz. The sightseeing flight had departed from Hohenems airfield on
Friday afternoon. The wreckage is lying in 40-50 metres of water,
making the divers' task more demanding.

Met officers badly injured in dog attack
Four Metropolitan Police officers participating in Operation Big
Wing to trace people wanted by the police or wanted in connection with
non-court appearance have been admitted to Royal London Hospital after
being attacked by a pit-bull dog while apprehending a 25-year old
suspect at an address in Albert Square, Newham.
Armed officers of the CO19 Specialist Firearms Command attended the
incident and shot the dog which had inflicted serious bite injuries to
5 officers in total.

Body lowered into wrong lair in Cumbrian cemetery
Copeland Council in Cumbria has
taken disciplinary action against a member of staff of Whitehaven
Cemetery after a body was lowered into the wrong lair during a funeral
there. The error was spotted prior to a full burial taking place,
precluding the need for an exhumation order from the Ministry of

A 63-year old painter has died in a fall from the parapet of a
building while working in Trinidad in the Flores region of south
Uruguay. The accident prompted workers to strike in some localities
yesterday in protest over laxity in safety culture.

Elderly Wexford resident lay dead for 3 months
A post-mortem has been undertaken at Waterford Regional Hospital where
the decomposed body of a British male in his 60's was taken after
being found in a house on Lower John Street, Wexford.
The alert was raised by a passer-by who noticed a decorated Xmas tree
visible in the property in which the resident is speculated to have
lain dead for 3 months.

Recovery operation at Hercules crash site on Mt Kebnekaise
Statens Haverikommission, the Swedish Accident Investigation
Authority, has indicated that rescuers have recovered human remains
near the site of a crashed Hercules military aircraft on Mount
Kebnekaise. The risk of avalanche on Sweden's highest peak has posed
risks for rescuers, it remains unclear why the C-130 J Super Hercules,
manufactured by Lockheed Martin of America and carrying 5 personnel,
lost altitude and crashed. It was participating in the Cold Response
military training exercise.

In Carpi, Emilia Romagna, the proprietor of a transport company died
in an accident when he was struck by a lorry; at Civitella Messer
Raimundo, Abruzzo, a 78-year old farmer was killed when he was struck
by a 10-metre high oak tree being felled on his land.

Balcony collapse brings scaffold down
Three workers aged 20,36 and 46 were hospitalised in Dachau at the
weekend after a fall from height accident at a housing site in
Olching, 10 miles northwest of Munich. A balcony which had just been
completed collapsed and caused their scaffold to fall.

Diver rescued from Channel shipping lane
Dover Coastguard co-ordinated the rescue of a diver yesterday after
the latter was spotted in the shipping lane by a ship passing the
Sandettie South West Buoy in the Dover Straits. He was later safely
returned to his dive boat. Dover Coastguard was unaware of his dive
plans and stress the importance of advising the coastguard in advance
of any plans to dive.

A 12-year old Essex schoolgirl died yesterday after becoming unwell
during a PE lesson at Woodland Comprehensive School, Basildon. She was
participating in a rugby lesson and she was attempting to catch a ball
which struck her chest. She fell over and appeared to experience a

Inadequately trained worker injured on defective machine
At Lincoln Crown Court DS Smith Packaging of Louth was fined £50K plus
£8,244 in costs over an accident in which a worker operating a machine
which rolled up corrugated packaging sustained a multiple fracture of
an arm and cracked ribs when he was thrown backwards as he operated
it. Investigation established that he had been trained to use the
machine by a worker who had not completed full training and the
machine was defective and had a design fault.

West country hotel fined over toddler's fall
At Bath Magistrates' Court The Old Mill Hotel was fined £10K plus
£1,948 in costs over an accident in which a 2-year old girl who was a
guest at a wedding fractured her skull in a 3-metre fall from a first
floor window.
A official of Bath and North East Somerset Council commented that
safety measures in place at the window were inadequate.

Seven people were admitted to the Erasmus hospital in Rotterdam
yesterday for treatment for carbon monoxide poisoning after fumes
built up in a building in the Delfshaven area of the city in which a
generator was running.

CO accident in tent
A 49-year old camper was listed in critical condition in Borders
General Hospital following a carbon monoxide accident while camping on
Minch Moor. He was reported missing and was found 24 hours later in
his tent in an oxygen-deficient atmosphere, the cause attributed to
gas-powered cooking or lighting apparatus.

Unfooted ladder accident
At Albany Magistrates' Court Ferngrove Vineyards was fined
$60K(£40,260) after pleading guilty to failing to provide a safe
workplace in respect of an accident in which a worker was injured in a
3-metre fall from an extension ladder while cleaning the outside of a
6-metre high wine vat.
A WorkSafe inspector observed that the accident was avoidable if a
colleague had assisted by footing the ladder as described in the
company's health & safety manual.

Svalbard governor's verdict on British schoolboy's mauling by polar
bear
Following an accident in which a British schoolboy was mauled to
death by a polar bear last August while at a British Schools Exploring
Society camp at the Von Postbreen glacier, 25 miles from Longyearbyen,
the governor of Svalbard has indicated that no charges of criminal
negligence are to be pursued.
The bear breached a tripwire which failed to warn of its presence and
a rifle failed to discharge adequately to repel the attack which also
saw 4 others injured.
The parents of the deceased Etonian scholar are expected to appeal the
governor's verdict.
