Italy 5 divers died in 2 separate accidents Five recreational divers have died in 2 separate incidents yesterday. Four divers in a party of 15 died after getting into difficulties in the 15-metre deep Grotta del Sangue, one of 35 caves at Capo Palinuro, south of Salerno. A Neapolitan diver died in a separate accident while diving at Capo Tiberio off Capri. 30th June 2012 Guyana Soldiers killed in Operation Harpie Two soldiers of the 9th RIMa were killed and 2 policemen were injured when illegal gold miners returned fire as the officers were participating in Operation Harpie to control illegal gold mining and panning in Dorlin in southwest Guyana where Brazilian "garimpeiros"(miners) are believed to be active. One of the policemen has abdominal wounds, the other was shot in the arm and leg.  29th June 2012 Saudi Arabia Train derailment under investigation Saudi Railways Organisation is investigating the derailment of a Dammam-Riyadh train near Al-Khurais Junction which left 35 people injured. Initial speculation was that heavy rolls of cloth stacked against the wall may have contributed to the collapse. 25th June 2012 Italy Illegal immigrants found dead on Adriatic ferry Two illegal immigrants were found dead on the car deck of a Superfast Adriatic ferry as it arrived in Ancona from Greece. Eighteen immigrants had apparently concealed themselves under buses and lorries, enduring conditions of searing heat. Three others were hospitalised in coma condition. The flight from Mazuco in Madre de Dios was making for Cusco with 2 crew members and 14 passengers. Snowy conditions were hampering the aerial search and rescue mission but teams were being mobilised on the ground. 8th June 2012 Canada Winnipeg window cleaner falls 8 storeys A window cleaner is listed in critical condition in Winnipeg Hospital after falling 8 storeys at premises on Cambridge Gardens in the River Heights district of the city. Initial speculation suggests that his rope failed, he fell through a solarium at ground level which enabled him to survive the fall. 8th June 2012 Russia 9 Tuva paratroopers dead in forest crown fire The Russian Federal Forestry Agency has reported that 9 paratroopers are feared dead while fighting a forest fire in Barum-Khemchiksky in Tuva. Thirteen service men had been brought in as part of the fire-fighting operation in Tuva, 4 are known to have survived. All the men had undergone specific training in such fire-fighting but a severe crown fire involving canopy burning is believed to have developed. 7th June 2012 Argentina Falling window panes kill worker during unloading A 28-year old Paraguayan worker has died in a crush accident at Complejo Villa Huapi of Sur SRL in Bariloche where he was crushed under an unstable load of windows weighing 400Kg during unloading from a lorry of Empresa Transportes Cruz del Sur from Buenos Aires. 7th June 2012 Switzerland Rock slippage leaves worker trapped A worker engaged on work securing the rock face above the Gotthard Railway has been buried under tonnes of rock when 2-3Km³ of rock and debris became unstable and slid at a work site in Gurtnallen yesterday. Two of his colleagues were rescued safely but the potential instability of a further 500m³ of earth and rock meant that further rescue efforts were impossible without the aid of Geo-radar equipment to monitor rock movement at  the site. Complications occurred during surgery which saw a lung spasm develop, reducing oxygen intake and she failed to awake from the anaesthesia. The anaesthetic is believed to have contained propofol, local anaesthetic was considered unsuitable given the extent of caries in the girl's teeth.   20th June 2012 Malaysia Workers' hostel fire at duck farm It is speculated that a candle caused the fire in a hostel building for workers at a duck farm in Jalan Minyak Beku in Batu Pahat district which led to the death of a female worker. The precarious nature of the situation and his injuries necessitated rescue by specialist group GRIMP 83( Groupe de Reconnaissance et d'Intervention en Milieux Perilleux). He was in intensive care last night after surgery for spinal injuries. 13th June 2012 Spain Aragon construction worker killed in fall from scaffold A construction worker of LAM died after breaking his neck in a 3-metre fall from scaffolding during refurbishment work on a building on Amargura Street in the old quarter of Teruel. 12th June 2012 Bolivia 18 dead in mountain ravine bus crash Eighteen people died and 41 were injured when a Trans Arenas bus left the road during a passing manoeuvre on a narrow section and tumbled down a 150-metre steep embankment near Unduavi while carrying  a party of teenagers from the Unidad Educativa of Chicaloma to the Juegos Plurinacionales Estudiantes sports event in La Paz. They were pruning poplar canopy which was encroaching on power lines when the MEWP suddenly lurched downwards, neither man was wearing a safety harness and they were pitched out of their box. 27th June 2012 Serbia Production press explosion in explosives factory An explosion occurred on Monday at the Milan Blagojecic factory in Lucani when a press used in the manufacture of rocket fuel and gunpowder exploded. Eight workers were on the premises at the time but no one was directly at the locus, although 1 worker sustained light injuries. 26th June 2012 Romania Pilot and passengers survive air crash An investigation is ongoing into the crash of a Cessna 206 aircraft in Cornetu, southwest of Bucharest, all 4 occupants survived with minor injuries after the aircraft apparently experienced engine failure. 26th June 2012 India Bangalore worker buried alive in muddy trench A subcontract construction worker was buried alive in an unstable excavation on Saturday evening at the Bangalore Water Supply and Sewerage Board site on Bannerghatta Road, Bangalore. A mass of silty material deposited beside the trench also slid back in when the instability occurred, workers required an excavator to recover the worker's body. Charges are expected to be laid against the site supervisor under Section 304A of the Indian Penal Code, causing death by negligence. 25th June 2012 Germany Teenager's finger severed in press At the Hirschvogel company in Denklingen, Bavaria, a 19-year old worker had a section of finger cut off in a hydraulic press while cleaning the machinery. He was trying to clean in an area which was barely accessible through a narrow aperture when his finger contacted a part of the machinery which was still active. 25th June 2012 Canada New Brunswick site fatality A worker died in an accident on a construction site on Grasse Circle in Fredericton on Thursday, details of the accident were not known. 22nd June 2012 England Tricky rescue of church tower scaffolder Suffolk fire crews had to abseil down the tower of St Nicholas Church in Great Yarmouth to rescue a 25-year old scaffolder who slipped and fell on to a flat roof while erecting a scaffold. 21st June 2012 Scotland Demolition gas cylinder exploded at power station The HSE has been advised of an accident at Inverkip Power Station where a gas cylinder exploded during the removal of cylinders by Brown and Mason, subcontracting to Scottish Power. A 40-year old demolition worker escaped with light injuries, the explosion was not related to demolition work. 21st June 2012 Germany Rescuing worker created further trench collapse Two Bavarian site workers were injured yesterday in an unstable excavation collapse on a site in Sengenthal. In the initial partial collapse of the trench a 50-year old worker became trapped up to his thighs in soil. A 56-year old excavator driver jumped in to help rescue him but this created further soil ingress which saw the latter trapped to his midriff. The latter worker was hospitalised in Neumarkt and is believed to have broken vertebrae in his spine. 20th June 2012 Rep Ireland Air ambulance in heavy landing after hitting wires The Air Accident Investigation Unit has been advised of an accident on Tuesday in which an Air Corps Eurocopter, a new emergency air ambulance service, had to make a heavy landing at Borissoleigh, County Tipperary, after hitting overhead wires. The helicopter, on call-out for a patient, is based at Custume Barracks, Athlone.  20th June 2012 Romania Transylvanian workers badly burned in mine boiler explosion A 43-year old manager and a 41-year old engineer were admitted to the burns unit of a hospital in Bucharest yesterday evening after being sprayed by boiling water in a boiler explosion at the Petrila mine in Jiu Valley. They were treated initially in a clinic in Petrosani prior to transfer by SMURD(Serviciul Mobil de Urgenta Reanimare si Descarcerare) to Bucharest. The senior worker has grade 3 and 4 burns over 60% of his body, the engineer has 40% burns. An explosion at the mine in 2008 saw 12 miners killed. 19th June 2012 Switzerland Worker's legs jammed in sawmill machine A 21-year old sawmill worker had to be free from machinery in which his legs were jammed on Monday in the saw mill in Wolfertswil, it is believed that he had been clearing a blockage of off-cuts when the machinery re-activated. The extent of his injuries were not known. 19th June 2012 Scotland Diver dies in accident off Eyemouth A diver has died in hospital in Edinburgh after getting into difficulties while diving with a colleague off Eyemouth, the latter diver was safely rescued and transferred to land by lifeboat. 18th June 2012 New Zealand Heavy equipment fell from forklift and killed worker At Christchurch District Court Trans Diesel was fined $55K(£27,685) and ordered to pay $71,030(£35,750) compensation over an accident last September in which a worker was crushed by a generator which fell from a forklift truck. Earlier in the day 2 generators had been safely loaded on to a lorry but on the 3rd occasion instability occurred and the load fell on the worker who was standing between the forklift and lorry. Department of Labour investigation found that the company had failed to take all reasonable steps to ensure the safety of the worker, that the generator was too heavy for the forklift and that the worker had found himself in an area in which he should not have been during the loading. 15th June 2012 Switzerland Fall from ladder death at Zurich warehouse A 59-year old maintenance worker was killed yesterday in a 2-metre fall from a ladder while working at a high-bay warehouse in Buchs in the Zurich canton. The ladder was being footed by a colleague but it is thought that it was struck by the basket of a MEWP(mobile elevating work platform) being raised separately at the time. 14th June 2012 USA Worker dies in fall off runaway asphalt roller A municipal worker of the City of Willoughby in Ohio has died in an accident involving an asphalt roller in Strawberry Lane. The roller's drive chain failed as he was negotiating a gradient and he sustained fatal head injuries in a fall from the moving equipment. 14th June 2012 France Martinique schoolboy injured at work experience Heavy metal material fell and pinned a schoolboy of Petit Manoir school of Lamentin, in the municipal area of Fort-de-France, as he was participating in work experience at premises in Riviere-Pilote in south Martinique. He became pinned between equipment and a wall and heavy lifting gear was used to free him. His condition in hospital is not life-threatening. 13th June 2012 Serbia Choir's bus overturns in Branicevo At least 15 people were injured when a  bus of the Bantus company of Obrenovac carrying members of a church choir from Belgrade to a monastery in Petrovac na Mlavi overturned near its destination yesterday, some of the injured are listed in serious but stable condition. 12th June 2012 Georgia Falling wall kills worker on Batumi site A 50-year old worker has died when a wall collapsed during dismantling at the Magnolia housing construction site in Batumi. 11th June 2012 Dubai 2 workers die in labour camp roof collapse Two workers died and another was admitted to Rashid Hospital when the roof of a building at a labour camp collapsed in Al Quoz Industrial Area 2. Four maintenance workers have been arrested over allegations that maintenance company work permits may not have been authorised. 11th June 2012 Malta Pipe falls during crane lifting at cement works A heavy pipe fell during hoisting by crane yesterday at a cement works on Triq L-Industrija(Industry Street) in Kirkop, killing a 45-year old Nigerian worker. 8th June 2012 Wales Boy dies at closed quarry site An investigation has been launched into the death of a 14-year old Carmarthenshire boy who died in Cilyrychen Quarry in Llandybie, the Tarmac West quarry is a closed site but is alleged to be accessible to locals. 8th June 2012 USA Industrial press fatality in Michigan A Michigan worker has died in a crush accident involving a press at NuPak Solutions in Norton Shores. It is thought that he was attempting to remove material from the press when his head and upper body became trapped as the press activated. 7th June 2012 Australia Conveyor belt death at Queensland quarry Workplace Health & Safety Queensland is investigating a fatal accident at a quarry at Moranbah where a 21-year old worker has died in a conveyor belt accident. 7th June 2012 Germany Concrete pump boom touches power line A construction worker has been hospitalised after sustaining an electric shock during a concrete pour for a pavilion at a sports ground in Niederkassel on Tuesday. He was operating the handle of a concrete pump when the pump's steel boom contacted an overhead power line. 6th June 2012 Romania Bus crash driver may have fallen asleep It is speculated that the driver of a bus carrying Ukrainian tourists which crashed in Vrancea may have fallen asleep at the wheel. One tourist has died and 16 others were injured, a woman's arm was amputated at the locus to release her from the wreckage. 6th June 2012 Benin Crane collapses at hospital site A crane cable failed during the lifting of concrete at the site of the new Accident and Emergency ward at Benin Central Hospital in Edo State, causing the crane to collapse. Two workers were rescued safely from the rubble but there were fears that an engineer may still have been trapped. 5th June 2012 Northern Ireland Cargo boat skipper jailed over ferry collision At Downpatrick Crown Court the 55-year old Polish skipper of the cargo ship Union Moon was jailed for a year following a trial over the collision in March of his boat and the Stena Feronia car ferry in Belfast Lough. The skipper was found to have been 3 times over the alcohol limit, the Maritime & Coastguard Agency also charged the skipper with failing to keep a proper look-out and failing to navigate. 1st June 2012

Three boats were on the river when one of them carrying 8 soldiers overturned in the fast-flowing river. The worker had not received information, instruction or training in the use of the drill, and the work had no risk assessment, with no formal system in place to ensure the drill was guarded. 19th June 2012 Japan Boy injured in fall from rollercoaster Investigators are trying to establish why a 6-year old boy fell from the moving Dragon rollercoaster ride at Karuizawa Toy Kingdom in Tsumagoi, Gunma. He was injured in his fall of 1.5 metres on to concrete and it is unclear whether the seatbelt was loose or he was able to loosen it. It has been alleged that he exceeded the ride maximum height restriction by 20%. 19th June 2012 England Stock car race driver killed in race accident The HSE is investigating a fatal accident at Mildenhall Stadium in West Row, Suffolk, where a 36-year driver participating in the Saloon Stock Car British Championship died in an accident after his car struck a central reservation and rolled out of control during the early stage of the race. 18th June 2012 England Worker's top snagged in unguarded machine At Kirklees Magistrates Court, Huddersfield, P Craven Engineering Ltd was fined £5K plus £2,302 in costs over an accident involving unguarded machinery at its Heckmondwike factory. On 7th September 2010, a 33-year old worker's clothing snagged on an unguarded CNC machine which he was preparing to clean after a cycle in which a machine tool had been rotating at 300 rpm. The machine's interlocks, which would have de-activated the equipment as he entered it, were disabled, allowing it to run unguarded. The worker's hood top snagged on the machinery casing, injuries included cuts and abrasions and a cracked foot bone.15th June 2012 Switzerland Giant tyre explodes during inflation A mechanic was hospitalised yesterday with severe head injuries after being thrown against a steel column at a garage on Zurcher Street in Frauenfeld when an HGV tyre exploded during inflation. 14th June 2012 England Schoolgirl's gym death ruled sudden adult death syndrome A coroner has delivered a verdict of sudden adult death syndrome in respect of a fatality at Mid Suffolk Leisure Centre where a 15-year old girl pupil of Stowmarket High School died after a gym lesson. She had been performing somersaults on a trampoline when she became unwell, post-mortem examination had failed to establish cause of death. 13th June 2012 Greece British tourist dies in base jump accident A 42-year old English tourist died during base jumping at Navagio Beach on Zante when his parachute snagged on cliffs leaving him suspended. Emergency services tried to rescue him during a 3-hour ordeal before he fell. 13th June 2012 England Fall on outing led to care home resident's death A 79-year old female resident of Hartlands Rest Home, Shrewsbury, fell during an outing to shops with a carer in March 2011. She was using a walking frame but fell and banged her head, succumbing to a brain haemorrhage and pneumonia 4 days later in hospital. Staff had been instructed that she was not to be taken out unless in a wheelchair. At Shrewsbury Magistrates' Court the Home owners pleaded guilty to 2 breaches of the Health & Safety at Work etc Act 1974 by failing to implement measures to ensure the safety of residents and failing to undertake suitable and sufficient assessment of risks associated with escorting residents on trips from the premises. The case was referred to Shrewsbury Crown Court for sentencing as the Magistrates' Court maximum penalty of £40K was deemed insufficient. 12th June 2012 England Girl,4, injured in fall from height at adventure park The HSE is investigating an accident at the Chessington World of Adventure where a 4-year old girl fell more than 4 metres from the Tomb Blaster ride queue. She sustained a head injury and broken ribs and is listed in critical condition in St George's Hospital in south London. 11th June 2012 Northern Ireland Down farm worker falls into slurry tank The HSENI has been advised of a fatal accident on a farm on Redhill Road, Dromore, County Down, where a worker died after falling into a slurry tank. 8th June 2012 France English driven car hits Breton cycle group Five members of the ECTL cycle group from Lamball were injured when they were struck by a car being driven by an English driver as they were on a bend on the Cote des Granges near Moncontour, one of the cyclists is understood to be severely injured. 7th June 2012 Italy Naples workers sprayed with boiling oil Three construction workers were admitted to the burns unit of Carderalli Hospital in Naples on Tuesday after being sprayed by boiling oil when a hydraulic piston failed on equipment on a site on the city's Piazza Garibaldi. 6th June 2012 Poland Level crossing collision A 24-year old woman has died in a level crossing accident at Kobyla Laka in central Poland where a car was struck by a train travelling from Bydgoszcz to Katowice, the 26-year old car driver was taken to hospital.  6th June 2012 Scotland 6 cases of Legionnaires Disease in Edinburgh Six cases of Legionnaires' Disease have been confirmed in Edinburgh, with 4 other suspected cases under observation. Five of the patients are in intensive care, the 6th is in a high dependency unit. The focus of the investigation into the source of the outbreak is on industrial water cooling towers in the Gorgie, Dalry and Saughton districts of the city. 5th June 2012 Scotland Shetland fishing crew rescued Shetland Coastguard co-ordinated the rescue of the crew of the fishing boat Valhalla which ran aground and began taking in water on Thursday at West Linga off Whalsay, the crew was picked up by Whalsay Sailing Club. 1st June 2012
