accidents
Five recreational divers have died
in 2 separate incidents yesterday. Four divers in a party of 15 died
after getting into difficulties in the 15-metre deep Grotta del
Sangue, one of 35 caves at Capo Palinuro, south of Salerno.
A Neapolitan diver died in a separate accident while diving at Capo
Tiberio off Capri.

Soldiers killed in Operation Harpie
Two soldiers of the 9th RIMa were
killed and 2 policemen were injured when illegal gold miners returned
fire as the officers were participating in Operation Harpie to control
illegal gold mining and panning in Dorlin in southwest Guyana where
Brazilian "garimpeiros"(miners) are believed to be active. One of the
policemen has abdominal wounds, the other was shot in the arm and leg.

Dammam-Riyadh train near Al-Khurais Junction which left 35 people

The driver of a delivery
lorry was swept away to his death after his vehicle fell down into the
River Glatt, a tributary of the Rhine, at Niederglatt on Monday morning. He was driving along an

3 killed in wall collapse at textile works
A wall collapsed at the Leojin Sewing Company premises in Dong Van
Industrial Park in Ha Nam Province, killing 3 workers and injuring 4
others. Initial speculation was that heavy rolls of cloth stacked
against the wall may have contributed to the collapse.

Illegal immigrants found dead on Adriatic ferry
Two illegal immigrants were found dead on the car deck of a Superfast
Adriatic ferry as it arrived in Ancona from Greece. Eighteen
immigrants had apparently concealed themselves under buses and
lorries, enduring conditions of searing heat. Three others were
hospitalised in coma condition. Police were questioning 2 Hungarian
lorry drivers as they investigate the incident.

A 19-year old worker died in an accident involving the collision of 2

An explosion and fire at the Rashtriya Ispat Nigam Ltd steel plant in
Visakhapatnam has killed 8 workers and left 15 others injured, many in
critical condition. The accident occurred in an oxygen control unit

A Helicusc (Helicopteres del Cusco) S-58ET helicopter is missing and

A window cleaner is listed in critical condition in Winnipeg Hospital
after falling 8 storeys at premises on Cambridge Gardens in the River
Heights district of the city. Initial speculation suggests that his

9 Tuva paratroopers dead in forest crown fire
The Russian Federal Forestry Agency has reported that 9 paratroopers
are feared dead while fighting a forest fire in Barum-Khemchiksky in
Tuva. Thirteen service men had been brought in as part of the

men had undergone specific training in such fire-fighting but a severe

Rock slippage leaves worker trapped
A worker engaged on work securing the rock face above the Gotthard
Railway has been buried under tonnes of rock when 2-3Km³ of rock and
debris became unstable and slid at a work site in Gurtnallen
yesterday. Two of his colleagues were rescued safely but the potential
instability of a further 500m³ of earth and rock meant that further
rescue efforts were impossible without the aid of Geo-radar equipment
to monitor rock movement at the site. Further work to reach the
buried worker will begin today (Wednesday).

A scrap recycling worker has died in an accident at the Al Qusais
waste dump in Dubai where most of the municipality's waste is dumped.
The victim's job was to collect scrap from waste deposited by lorries

Girl fails to revive from dental anaesthesia
After several days in a coma following dental surgery for the
extraction of 10 teeth a 4-year old girl has died in hospital in Graz.
Complications occurred during surgery which saw a lung spasm develop,
reducing oxygen intake and she failed to awake from the anaesthesia.
The anaesthetic is believed to have contained propofol, local
anaesthetic was considered unsuitable given the extent of caries in
the girl's teeth.

injuries necessitated rescue by specialist group GRIMP 83( Groupe de
Reconnaissance et d'Intervention en Milieux Perilleux). He was in

MEWP (mobile work elevating platform). They were pruning poplar canopy
which was encroaching on power lines when the MEWP suddenly lurched
downwards, neither man was wearing a safety harness and they were
pitched out of their box.

Bangalore worker buried alive in muddy trench
A subcontract construction worker was buried alive in an unstable
excavation on Saturday evening at the Bangalore Water Supply and
Sewerage Board site on Bannerghatta Road, Bangalore. A mass of silty
material deposited beside the trench also slid back in when the
instability occurred, workers required an excavator to recover the
worker's body.
Charges are expected to be laid against the site supervisor under
Section 304A of the Indian Penal Code, causing death by negligence.

At the Hirschvogel company in Denklingen, Bavaria, a 19-year old worker had a
section of finger cut off in a hydraulic press while cleaning the
machinery. He was trying to clean in an area which was barely

Air ambulance in heavy landing after hitting wires
The Air Accident Investigation Unit has been advised of an accident on
Tuesday in which an Air Corps Eurocopter, a new emergency air
ambulance service, had to make a heavy landing at Borissoleigh, County
Tipperary, after hitting overhead wires. The helicopter, on call-out
for a patient, is based at Custume Barracks, Athlone.

Transylvanian workers badly burned in mine boiler explosion
A 43-year old manager and a 41-year
old engineer were admitted to the burns unit of a hospital in
Bucharest yesterday evening after being sprayed by boiling water in a
boiler explosion at the Petrila mine in Jiu Valley. They were treated
initially in a clinic in Petrosani prior to transfer by
SMURD(Serviciul Mobil de Urgenta Reanimare si Descarcerare) to
Bucharest. The senior worker has grade 3 and 4 burns over 60% of his
body, the engineer has 40% burns.
An explosion at the mine in 2008 saw 12 miners killed.

Worker's legs jammed in sawmill machine
A 21-year old sawmill worker had to be free from machinery in which
his legs were jammed on Monday in the saw mill in Wolfertswil, it is
believed that he had been clearing a blockage of off-cuts when the
machinery re-activated. The extent of his injuries were not known.

Heavy equipment fell from forklift and killed worker
At Christchurch District Court Trans Diesel was fined $55K(£27,685)
and ordered to pay $71,030(£35,750) compensation over an accident last
September in which a worker was crushed by a generator which fell from
a forklift truck. Earlier in the day 2 generators had been safely
loaded on to a lorry but on the 3rd occasion instability occurred and
the load fell on the worker who was standing between the forklift and
lorry. Department of Labour investigation found that the company had
failed to take all reasonable steps to ensure the safety of the
worker, that the generator was too heavy for the forklift and that the
worker had found himself in an area in which he should not have been
during the loading.

Fall from ladder death at Zurich warehouse
A 59-year old maintenance worker was killed yesterday in a 2-metre
fall from a ladder while working at a high-bay warehouse in Buchs in
the Zurich canton. The ladder was being footed by a colleague but it
is thought that it was struck by the basket of a MEWP(mobile elevating
work platform) being raised separately at the time.

A municipal worker of the City of Willoughby in Ohio has died in an
accident involving an asphalt roller in Strawberry Lane. The roller's
drive chain failed as he was negotiating a gradient and he sustained

Martinique schoolboy injured at work experience
Heavy metal material fell and pinned a schoolboy of Petit Manoir
school of Lamentin, in the municipal area of Fort-de-France, as he was
participating in work experience at premises in Riviere-Pilote in
south Martinique. He became pinned between equipment and a wall and
heavy lifting gear was used to free him. His condition in hospital is
not life-threatening.

At Downpatrick Crown Court the 55-year old Polish skipper of the cargo
ship Union Moon was jailed for a year following a trial over the
collision in March of his boat and the Stena Feronia car ferry in
Belfast Lough. The skipper was found to have been 3 times over the
alcohol limit, the Maritime & Coastguard Agency also charged the
skipper with failing to keep a proper look-out and failing to

A 22-year old serviceman with 2 Logistic Support Regiment has drowned

River Inn in Austria. Three boats were on the river when one of them

soldiers were swept away in the strong current but 2 of them managed
to make it to the river bank.

At Abergavenny Magistrates' Court MIB Manufacturing has been fined
£15K plus £9,9947 in costs and £3K in compensation over an accident in

premises in Blaina, Blaenau Gwent. The worker had not received
information, instruction or training in the use of the drill, and the
work had no risk assessment, with no formal system in place to ensure
the drill was guarded.

establish why a 6-year old boy fell from the moving Dragon
rollercoaster ride at Karuizawa Toy Kingdom in Tsumagoi, Gunma. He was

whether the seatbelt was loose or he was able to loosen it. It has

Worker's top snagged in unguarded machine
At Kirklees Magistrates Court, Huddersfield, P Craven Engineering Ltd
was fined £5K plus £2,302 in costs
over an accident involving unguarded machinery at its Heckmondwike
factory.
On 7th September 2010, a 33-year old worker's clothing snagged on an
unguarded CNC machine which he was preparing to clean after a cycle in
which a machine tool had been rotating at 300 rpm. The machine's
interlocks, which would have de-activated the equipment as he entered
it, were disabled, allowing it to run unguarded. The worker's hood top
snagged on the machinery casing, injuries included cuts and abrasions

Fall on outing led to care home resident's death
A 79-year old female resident of Hartlands Rest Home, Shrewsbury, fell
during an outing to shops with a carer in March 2011. She was using a
walking frame but fell and banged her head, succumbing to a brain
haemorrhage and pneumonia 4 days later in hospital. Staff had been
instructed that she was not to be taken out unless in a wheelchair.
At Shrewsbury Magistrates' Court the Home owners pleaded guilty to 2
breaches of the Health & Safety at Work etc Act 1974 by failing to
implement measures to ensure the safety of residents and failing to
undertake suitable and sufficient assessment of risks associated with
escorting residents on trips from the premises. The case was referred
to Shrewsbury Crown Court for sentencing as the Magistrates' Court
maximum penalty of £40K was deemed insufficient. 12th June 2012

6 cases of Legionnaires Disease in Edinburgh
Six cases of Legionnaires' Disease have been confirmed in Edinburgh,
with 4 other suspected cases under observation. Five of the patients
are in intensive care, the 6th is in a high dependency unit.
The focus of the investigation into the source of the outbreak is on
industrial water cooling towers in the Gorgie, Dalry and Saughton
districts of the city.
