India Dozens killed in train fire At least 47 passengers have died when fire broke out in a carriage on the overnight New Delhi- Chennai express train as it passed through Nellore, 300 miles south of Hyderabad. It is speculated that an electrical malfunction caused the fire. He was moving a continuous mixer but became caught between the machine and the mine wall. This was the 3rd state mining death of 2012. 29th July 2012 Russia Miners die in methane explosion Two accidents at Kemorovo mines yesterday saw 3 miners killed. The fatalities occurred in a methane explosion at the Ziminka mine in the Prokopyevsk district. Fire broke out at the No3-Tolmachevsky coal bed at the Komsomolets mine in Lenisk-Kuzhnetsky where all 263 miners were safely evacuated. 27th July 2012 Trinidad and Tobago Valve blow out kills maintenance worker Petrotrin is investigating a fall from height fatality at a platform on its FCCU(Fluidised Catalytic Cracking Unit) plant at the Pointe a Pierre refinery where a temporary worker servicing a valve was struck in a blow-out accident. 27th July 2012 France 6 dead in helicopter test flight crash A Super Puma AS 532 AL helicopter has crashed in the La Maline area of La Palud sur Verdon with the loss of 6 lives, test pilots and engineers of Eurocopter. The helicopter is believed to have been on a test flight from a heliport near Marseille prior to delivery to a customer. It is speculated that the helicopter may have struck electricity cables. 26th July 2012 Austria Falling glass kills HGV driver and tyre fitter The 47-year old Romanian driver of a Slovakian lorry and a 37-year old Austrian tyre fitter died in an accident on the motorway near Peterskirchen. More than 30 people are known to have died and 145 have been rescued, but with an estimated 280 people on board there are fears that many more have drowned. The ferry, operated by the Seagull company, was sailing from the Tanzanian mainland to Zanzibar. 19th July 2012 Singapore Structural collapse during concrete pour The collapse of a temporary scaffold at the Bugis Station subway link yesterday has killed 2 workers and left 8 others injured. Elsewhere on Sunday, 8 miners were rescued after being trapped for 3 days in the flooded Qielichong Coal Mine in Sandu, Leiyang, strenuous efforts were ongoing last night to reach the remaining 8 miners. Within an hour a 2nd helicopter from Daroca had also crashed at Cortes de Pallas in the Sierra Martes, a 36-year old pilot is listed in stable condition but the condition of his 44 year old colleague is causing concern in La Fe Hospital, Valencia. 3rd July 2012 Botswana Roof collapse kills diamond miner A stop-work order was issued at the Jwaneng diamond mine of the Debswana Diamond Company, 75 miles west of Gaborone, where a partial roof collapse occurred, killing a 34-year old worker driving a vehicle underground. All other 15 miners underground survived the accident. 3rd July 2012 South Africa 5 gold miners die in fire at West Rand mine Five miners died and 14 others have been hospitalised in stable condition following a fire on the night shift on Saturday at 4Shaft(Ya-Rona) KDC West mine of GoldFields in Carletonville. The cemetery features many monuments to silver miners interred in the 19th century, it is thought that the boy had grasped a metal section attached to the stone and this failed, causing the stone to become unstable and fall on him, inflicting blunt force trauma to his upper body. 9th July 2012 New Zealand North Island man dies in grader accident A worker has died of head injuries when he had to jump clear from a road grading vehicle after losing control of it near Vernon Farms in Ramarama, Auckland Region. It is thought that he struck his head in attempting to leap to safety and was not struck by the overturning vehicle.  9th July 2012 Colombia Falling jack kills site worker A 12Kg hydraulic jack fell from the 12th storey of a building under construction in north Barranquilla, striking a construction worker on the 2nd floor. USA Rhode Island worker electrocuted A grounds maintenance worker of AAA Professional Lawn Care was electrocuted on Monday during arboreal work at a house on County View Lane in Johnston, Rhode Island, when he contacted  overhead power lines while working at height trimming tree canopy. 31st July 2012 Austria Cleaner falls through porch roof On Monday a worker engaged in cleaning fell through the roof of the canopy at a silo in Harmannsdorf, Lower Austria. He was carrying a 20-litre container of liquid and the fall has left him with several fractures and internal injuries. 31st July 2012 England Fall from height death A 66-year old man has died in hospital following an accident last week at South Coast Skips on the Rudford Industrial estate in Ford, West Sussex. He and a colleague were engaged in cabling work at the roof of a building and were raised up in the bucket of an excavator when they fell 10 metres. The judge observed that she should have been provided with a helmet and protective equipment. 27th July 2012 England Boy,11, grounded after flight to Rome Several staff of Manchester Airport and Jet2 have been suspended pending investigation of an incident in which an unaccompanied 11-year old boy boarded a flight to Rome with neither passport nor ticket. He was working in or on a large industrial skip, it was unclear whether he fell or was knocked over by an item of plant. A trench had been excavated to accommodate a water pipeline, for an unknown reason he had re-entered the trench which lacked shoring. 9th July 2012 The Netherlands Resident dies in smoke-logged house A 70-year old man died in a fire in a sheltered house on Jan Meijenstraat in Utrecht, his 65-year old wife was hospitalised. The fire was a relatively minor one but the smoke-logging was considerable. 9th July 2012 Germany Site worker dies under collapsed house founds A construction worker was killed in an accident at a house on Gutenburg Street in Ingolstadt on Thursday when the house foundations collapsed and concrete weighing 3 tonnes fell on him in a 2-metre deep excavation dug around the building to install a dry course. 6th July 2012 Luxembourg Accident at farm site At a farm in Hachiville where a new building is being erected a concrete section measuring 2m x 4m became unstable and fell, severely injuring a site worker on Thursday afternoon. 6th July 2012 England Worker crushed by falling granite At Chelmsford Crown Court The Stone Company Ltd was fined £20K plus £40K in costs over a fatal crush accident at its Great Baddow premises in Chelmsford where a 57-year old worker was crushed by falling granite on 28th April 2009. Manufactured stone was being unloaded at the warehouse and placed on A-frames. A pile weighing 3 tonnes became unstable and fell, crushing the worker against a wall. HSE investigation found the A-frames to be poorly sited and inappropriate within the confines of a warehouse. The company pleaded guilty to a breach of Section 2(1) of the Health & Safety at Work etc Act 1974. 5th July 2012 England Fall through roof light at farm At Beverley Magistrates' Court a farmer has been fined £6K plus £3,894 costs after pleading guilty to a breach of the Work at Height Regulations 2005. On 19th July 2011, a 21-year old worker fell though a fragile roof light while trying to effect roof repairs at Hunsley Farm in Little Weighton. He sustained a punctured lung and kidney and cracked 4 vertebrae. 5th July 2012 Scotland Tornado pilots missing after mid-air collision Life boats from Wick and Buckie were searching an area of the Moray Firth south of Wick for 2 pilots missing following the collision of 2 RAF Tornado GR4 jets from RAF Lossiemouth, the 2 other pilots were picked up from the water and taken to Raigmore Hospital, Inverness. Bad weather was hampering the search for the missing men on Tuesday evening. 4th July 2012 Austria Roofer fires nail through hand A roofer was hospitalised in Gussing, Burgenland, yesterday  with a serious hand injury after accidentally firing a nail from a nail gun through his hand while on the roof of a property in St Michael. l4th July 2012 The Netherlands HGV driver crushed at Botlek site A fatal accident investigation has been launched by the Dutch Arbeidsinspectie (work inspectorate) after a lorry driver was crushed yesterday between a lorry and a parked trailer at a work site on Merceyweg, Botlek. 3rd July 2012 Russia Zoo fined over tiger attack on child A court in Blagoveshchensk has awarded compensation of 300K roubles(£5,871) to a family whose 2-year old child was attacked by a tiger while visiting the Liger travelling zoo on 4th January this year. Charges were filed under Article 234 of the Russian Penal Code regarding negligence in respect of the  health and safety of visitors. The tiger was outside its cage and chained to a stake with a view to visitors taking photos in the vicinity. 3rd July 2012 England Worker's fingers severed cleaning concrete pump A worker had 4 fingers severed in an accident while cleaning a concrete hopper pump at a house site in Telford on 27th February 2010. He had lifted a grill which should have deactivated the machinery as a safeguard. The supplier of the pump, Transcrete (GB)Ltd was fined £4K plus £2K costs for failing to ensure the machine was designed and constructed in a manner that it could not cause serious injury. O' Neill Concrete Pumping Ltd of Wolverhampton was fined £3K plus £2K costs for failing to ensure the safety of an employee. 2nd July 2012

England Boys injured in Leics fairground accident The HSE is undertaking investigation of an accident at the Billy Bates fair in Abbey Park, Leicestershire, where 2 boys were injured while travelling on the Mega Bounce Frog ride. A carriage tipped over, pitching one of the boys against a barrier and leaving the other pinned under the overturned carriage. The latter boy is in intensive care with chest injuries and a broken arm, the other had lesser injuries. He had entered the fragmentiser but the car slid downwards on to him, leaving him with fractures and kidney and liver damage. HSE investigation found that this was a monthly occurrence but staff had inadequate training for such an exigency. The staff member who was trained in the equipment's operation was on holiday at the time of the accident. The company pleaded guilty to a breach of Section 2(1) of the Health & Safety at Work etc Act 1974 by failing to have a safe system of work at its site on Griffon Road, Ilkeston. 25th July 2012 Scotland Diver missing in Irish Sea A number of lifeboats have been participating in a search and rescue mission in the Irish Sea for a diver who failed to return to a dive boat 10 miles north of Corsewall Point, Dumfries and Galloway. 24th July 2012 England Race gliders collide over Newmarket Two gliders collided while participating in the 3rd day of the 2012 British Club Class Gliding National Championships being run by Cambridge Gliding Centre at Gransden Lodge Airfield. One of the pilots ejected and was being treated for a head injury, the other was unhurt. 24th July 2012 England Boy dies in accident at Durham open-air museum A 7-year old boy died after sustaining severe head injuries following an accident yesterday involving a steam traction engine and trailer at the Beamish Museum near Stanley, County Durham. 23rd July 2012 Sweden Adult's rectal injury on child's ride at theme park A 31-year old Dalarna man was hospitalised for several days with blood poisoning after sustaining rectal injuries when he sat on a protruding tow bar on a children's ride at the Furuviksparken theme park. In attempting to help his daughter operate the mini truck on the Traffic Square theme he inadvertently sat on the tow hitch behind the seat. 23rd July 2012 Wales Unwell woman airlifted off gas tanker The Holyhead lifeboat attended an incident after a senior female officer on the Norwegian gas tanker Nancy Kutson became ill, after administering first aid she was airlifted by helicopter to hospital in Bangor. 20th July 2012 England Elderly patient died in fall from Bucks hospital balcony At Aylesbury Crown Court BMI has been fined £100K plus £29,446 in costs over the death of an 85-year old patient in a 5-metre fall from a balcony at the BMI Shelbourne Hospital in High Wycombe. The court heard that sliding doors affording access to the balcony could not be securely locked. 19th July 2012 England Worn valve failed in Yorks mine crush fatality UK Coal has been fined £200K at Leeds Crown Court over the death of a miner at Kellingley Pit in North Yorkshire where he was crushed by a powered roof support which lowered spontaneously, crushing him against accumulated debris. Investigation found that a valve in the support was worn and defective and that supports had been salvaged from another coal face and assessed by UK Coal as fit for transfer with limited maintenance. The supports were supplied by Joy Machinery Ltd who had failed to notify UK Coal of a similar malfunction in Australia in 2008. Joy was fined £50K at an earlier hearing. 19th July 2012 Indonesia Tiger kills Sumatran plantation worker An 18-year old female palm oil plantation worker has been killed by a tiger at the plantation in the village of Indragiri Hul in Riau, Sumatra.  18th July 2012 New Zealand Worker injured in lumber conveyor At Whakatane District Court the Sequal Lumber Ltd Partnership has been fined $20K(£10,187) plus $8K(£4,075) in compensation over an accident in which a worker sustained fractures after falling on to a conveyor system. He had been shovelling bark on to the conveyor but tripped over a dropped shovel and fell on to the system, his arm becoming trapped between the conveyor and pulley. The bus is undergoing inspection at the Dundalk depot.  11th July 2012 Scotland Crew rescued from burning trawler All 6 crew members of the Banff trawler Denarius were safely rescued from their trawler after a mayday was issued when it caught fire 60 miles east of Orkney. Shetland Coastguard airlifted them to safety, stressing how crucial the activation of their EPIRB(Emergency Position Indicating Radio Beacon) was in facilitating the rescue. 10th July 2012 England Lancs parachutist killed A male parachutist of the Black Knights Parachute Centre died in an accident at the weekend when his chute failed to activate properly as he participated in a jump at the Cockerham Gala in Lancashire. The parachute appeared to become entangled early into the jump and he attempted to cut away and detach the main chute to use the emergency chute, however he got too close to ground for the emergency chute to prove effective.  9th July 2012 England Candles ignited hair salon fumes An octogenarian was admitted to the burns unit at Whiston Hospital following an accident at the John Hemmingway Hair salon in Wallasey Village, Merseyside. She was receiving a manicure there but candles on her birthday cake ignited fumes in the premises. 9th July 2012s Northern Ireland Worker's arm trapped in bakery machine The fire service had to deploy hydraulic equipment to free a worker whose arm had become trapped in machinery while cleaning it at the Linwoods food company in Madden, County Armagh. The 29-year old worker was subsequently hospitalised, the severity of the injury was not known. 6th July 2012 Switzerland Zurich church hall roof falls during renovation Two building workers were hospitalised with leg and back injuries when the roof of a church hall at the Paulus church in Zurich collapsed on them during renovation work. 6th July 2012 England Test driver killed in vehicle testing accident A worker, test driving a car for Honda, was killed and 4 others were injured in a single vehicle crash at the Millbrook Proving Ground test track during routine vehicle testing. The injured workers were admitted to Bedford Hospital but are not in life-threatening condition, the HSE is participating in the investigation. 5th July 2012 England Falling metal breaks Berks worker's leg A worker's leg was broken when metal fell in a machine shop in Berkshire, he was airlifted from a field near Brookfield Farm, Wokingham, to hospital in Slough, there were fears initially that the leg may have been so badly injured that it might require amputation. 5th July 2012 England F1 test driver badly injured at Cambs airfield A 32-year old female test driver of the Marussia Formula 1 team was admitted to Addenbrooke's Hospital in Cambridge with life-threatening injuries yesterday after a car she was driving at Duxford airfield collided with the team's support lorry. 4th July 2012 England Council fined over death of disabled driver by reversing bin van Bassetlaw District Council has been fined £25K plus £12,987 in costs at Nottingham Crown Court over an accident during a round in Langold, Notts, in July 2008 when a mobility scooter driver was killed by a reversing bin lorry. The vehicle was on a missed bin collection and had a single-man crew, an HSE inspector observed that the accident could have been prevented if the driver had had a reversing assistant. The council pleaded guilty to a breach of Section 3(1) of the Health & Safety at Work etc Act 1974. 4th July 2012 Scotland Boy rescued unconscious from salt-water pool The Aberdeen shire Council salt-water swimming pool at Stonehaven has been closed while an investigation has taken place into an accident which  saw an 11-year old boy rescued unconscious from the pool and transferred to a specialist hospital unit in Edinburgh. 3rd July 2012 Rep Ireland Mayo nursing home resident found drowned A 68-year old resident of Ballinamore Nursing Home was found drowned in the Glore river near Kiltimagh, County Mayo, after being reported missing on Friday.
