Dozens killed in train fire
At least 47 passengers have died when fire broke out in a carriage on
the overnight New Delhi- Chennai express train as it passed through
Nellore, 300 miles south of Hyderabad. It is speculated that an
electrical malfunction caused the fire. Twenty-eight other passengers
have been hospitalised with burns, 2 are listed in intensive care.

Miners die in methane explosion
Two accidents at Kemorovo mines yesterday saw 3 miners killed. The
fatalities occurred in a methane explosion at the Ziminka mine in the
Prokopyevsk district. Fire broke out at the No3-Tolmachevsky coal bed
at the Komsomolets mine in Lenisk-Kuzhnetsky where all 263 miners were
safely evacuated.

A Super Puma AS 532 AL helicopter has crashed in the La Maline area of
La Palud sur Verdon with the loss of 6 lives, test pilots and
engineers of Eurocopter. The helicopter is believed to have been on a
test flight from a heliport near Marseille prior to delivery to a
customer. It is speculated that the helicopter may have struck
electricity cables.

Falling glass kills HGV driver and tyre fitter
The 47-year old Romanian driver of a Slovakian lorry and a 37-year old
Austrian tyre fitter died in an accident on the motorway near
Peterskirchen. The lorry had summoned assistance for a puncture and
both men were struck by 18 tonnes of glass plate which fell from
another lorry which had to brake hard in a traffic manoeuvre.

A 75-year old Campania farmer died in a crush accident yesterday on
his land at Fontana di Motta in Auletto, Salerno. He had stepped from
his tractor on to a trailer he was towing but the vehicle was
presumably unbraked and rolled down an embankment and overturned.

Scores feared drowned in ferry sinking
The ferry MV Skagit/Kalama capsized and sank in rough seas off Chumbe
island to the west of Zanzibar yesterday. More than 30 people are
known to have died and 145 have been rescued, but with an estimated
280 people on board there are fears that many more have drowned. The
ferry, operated by the Seagull company, was sailing from the Tanzanian
mainland to Zanzibar.

suspended in his harness at a height of 30 metres.

Shellmark Farms of Carberry, Manitoba, was fined $48,050(£30,365)
over an accident on 18th September 2010 in which a 15-year old worker

the moving conveyor. The company pleaded guilty to breaching Section

The Regional Aragon government has announced that a BRIF (Brigada de
Refuerzo de Incendios Forestales) died on Monday when his helicopter,
based at Daroca, Zaragoza, crashed at Forata, Valencia, during fire
fighting. Within an hour a 2nd helicopter from Daroca had also crashed
at Cortes de Pallas in the Sierra Martes, a 36-year old pilot is
listed in stable condition but the condition of his 44 year old

DS Down rescued the survivors from their lifeboat 1,000 miles west of
Cornwall. Winds were 3-4 Beaufort Scale, with a 1-metre sea swell.

Fall from height death
A 66-year old man has died in hospital following an accident last week
at South Coast Skips on the Rudford Industrial estate in Ford, West
Sussex. He and a colleague were engaged in cabling work at the roof of
a building and were raised up in the bucket of an excavator when they
fell 10 metres. The 21-year old colleague is recovering in hospital in
Chichester with fractures to his leg and elbow.

At Chelmsford Crown Court The Stone Company Ltd was fined £20K plus
£40K in costs over a fatal crush accident at its Great Baddow premises
in Chelmsford where a 57-year old worker was crushed by falling
granite on 28th April 2009. Manufactured stone was being unloaded at
the warehouse and placed on A-frames. A pile weighing 3 tonnes became
unstable and fell, crushing the worker against a wall. HSE
investigation found the A-frames to be poorly sited and inappropriate
within the confines of a warehouse. The company pleaded guilty to a
breach of Section 2(1) of the Health & Safety at Work etc Act 1974.

On 19th July 2011, a 21-year old worker fell though a fragile roof

Weighton. He sustained a punctured lung and kidney and cracked 4

A court in Blagoveshchensk has awarded compensation of 300K
roubles(£5,871) to a family whose 2-year old child was attacked by a
tiger while visiting the Liger travelling zoo on 4th January this
year.
Charges were filed under Article 234 of the Russian Penal Code
regarding negligence in respect of the health and safety of
visitors. The tiger was outside its cage and chained to a stake with a
view to visitors taking photos in the vicinity.

concrete hopper pump at a house site in Telford on 27th February 2010.
He had lifted a grill which should have deactivated the machinery as a

plus £2K costs for failing to ensure the machine was designed and
constructed in a manner that it could not cause serious injury.

Boys injured in Leics fairground accident
The HSE is undertaking investigation of an accident at the Billy Bates
fair in Abbey Park, Leicestershire, where 2 boys were injured while
travelling on the Mega Bounce Frog ride. A carriage tipped over,

pinned under the overturned carriage. The latter boy is in intensive
care with chest injuries and a broken arm, the other had lesser

Worker entered car crusher to clear blockage
At Southern Derbyshire Magistrates' Court Ward Recycling has been
fined £20K plus £19,970 in costs over an accident on 17th June 2009 in
which a worker was badly injured while attempting to clear a blockage
in fragmentiser car-crushing equipment. He had entered the
fragmentiser but the car slid downwards on to him, leaving him with
fractures and kidney and liver damage. HSE investigation found that
this was a monthly occurrence but staff had inadequate training for
such an exigency. The staff member who was trained in the equipment's
operation was on holiday at the time of the accident. The company
pleaded guilty to a breach of Section 2(1) of the Health & Safety at
Work etc Act 1974 by failing to have a safe system of work at its site
on Griffon Road, Ilkeston.

Worn valve failed in Yorks mine crush fatality
UK Coal has been fined £200K at Leeds Crown Court over the death of a
miner at Kellingley Pit in North Yorkshire where he was crushed by a
powered roof support which lowered spontaneously, crushing him against
accumulated debris. Investigation found that a valve in the support
was worn and defective and that supports had been salvaged from
another coal face and assessed by UK Coal as fit for transfer with
limited maintenance. The supports were supplied by Joy Machinery Ltd
who had failed to notify UK Coal of a similar malfunction in Australia
in 2008. Joy was fined £50K at an earlier hearing.

At Whakatane District Court the Sequal Lumber Ltd Partnership has been
fined $20K(£10,187) plus $8K(£4,075) in compensation over an accident
in which a worker sustained fractures after falling on to a conveyor
system. He had been shovelling bark on to the conveyor but tripped
over a dropped shovel and fell on to the system, his arm becoming
trapped between the conveyor and pulley. The company was found to be
in breach of Section 6 of the Health & Safety in Employment Act 1992
in failing to ensure the safety of a worker.

Council fined over death of disabled driver by reversing bin van
Bassetlaw District Council has been fined £25K plus £12,987 in costs
at Nottingham Crown Court over an accident during a round in Langold,
Notts, in July 2008 when a mobility scooter driver was killed by a
reversing bin lorry. The vehicle was on a missed bin collection and
had a single-man crew, an HSE inspector observed that the accident
could have been prevented if the driver had had a reversing assistant.
The council pleaded guilty to a breach of Section 3(1) of the Health &
