Boy killed in tractor rollover; level crossing collision

A 9-year old boy has died in a crush accident when he fell from the cab of a tractor which overturned on a gradient in Wolfsegg am Hausruck in Upper Austria.
Elsewhere in Upper Austria yesterday, a goods train collided with a car on an open level crossing in Spital am Pyhrn, a 74-year old female passenger in the back of the car died but both occupants in the front escaped with light injuries.

BC Sawmill explosion

An explosion at the Lakeland Sawmill owned by the Sinclar Group Forest Products in Prince George, British Columbia, has killed 1 worker and injured 23 others, 6 of whom are still in hospital with burns injuries. Most workers were attending a training course in the dining room when the explosion occurred in a main mill. Other workers were in the planer mill and 4 were in the yard. Fire crews were contending with compressed gases and sawmill dust as they tackled the blaze which took most of yesterday to subdue.

Nine miners died and 16 were injured, 4 in critical condition, following a gas explosion early on Monday at the Xingya coal mine at Urad in Bayannur City. Recovery and investigative work was drawing to a close late on Monday evening at the licensed mine.

23 killed in fabric factory collapse

The factory owner, contractor and 3 other people have been arrested following an accident at the weekend in which the 4-storey Shital Fibre blanket factory in the Focal Point Industrial area of Jalandhar collapsed, 23 workers died and 65 others were rescued, although it is feared that others may still be trapped under the rubble.

After 4 days trapped underground at the Luicho mine on Colta a miner died just at the point when rescue teams were freeing him and beginning to move him. His foot had been trapped under rocks and medics speculated that thrombosis travelled to his lung at this point. The miner had also been contending with low temperatures during his ordeal

Ontario utility fined over electrocution

In court in Walkerton, Ontario, Westario Power Inc has been fined $110K(£68,811) over a fatal accident on September 21st 2010 in which a worker was electrocuted in a substation in Port Elgin. Workers had attempted to de-energise equipment with a view to replacing a damaged insulator, however this had not been achieved and a worker contacted a live power source and was electrocuted.
Ministry of Labour investigation found that no current single line diagram existed to assist workers in developing a work plan, identify hazards and fully de-energise equipment. The company pleading guilty to a breach of the Occupational Health & Safety Act Section 25(2)(a), pleading guilty to failing to provide information, instruction and supervision to protect the health and safety of workers.

Paint shop explosion leaves apprentice dead

An apprentice has died from critical burns after an explosion occurred in the apprentice training workshop at the Liebherr work in Nenzing. Some other workers who were injured are being treated in hospitals in Feldkirch and Bludenz.
Schoolchildren from Nenzing were on the premises as part of a work experience visit and may have been among the injured. The dead worker had been transferred to a specialist burns unit over the border in Murnau, Bavaria. No work was being undertaken in the paint shop at the time of the explosion, making the cause of the accident unclear.

A construction worker died and 5 were reported missing when a building under construction collapsed yesterday during a concrete pour at a site on Proyektriruyemy Lane in south Moscow. It was alleged that an additional floor was being constructed at the high-rise building in breach of  that for which planning consent was granted and that criminal proceedings under Article 216, part 2, of the RF Criminal Code(breaches of safety regulations during construction work) were being undertaken.

A 24-year old worker died in a crush accident on Wednesday involving a paper and cardboard compressor at a supermarket on Innsbruck Street in Hall in Tirol, it is thought that he was attempting to clear a blockage with a metal rod when he lost his balance and fell in.
In Vienna on Wednesday a 27-year old worker died and a colleague aged 33 was injured in a 20-metre fall into a lift shaft at a 4-storery building on Weitlofgasse, both belong to the company which installed the lift.

Reversing lorry kills site worker

The Ministry of Labour has issued a stop work order at the site of a new condominium being built by Design Build in Thunder Bay after a  worker of LTL Contracting was killed by a reversing lorry.
Of specific relevance in the order is that reversing vehicles with an obstructed view must be guided by competent signallers and no equipment should be operated in reverse movement unless no practical alternative exists on site.

Brussels bus inspector dies in assault incident

A 59-year old Brussels bus inspector, an employee of STIB(Societe des Transport Intercommunaux de Bruxelles) died in the weekend after being struck by a single punch by an assailant after he attended the locus of a collision between a Line 14 bus and a car. A suspect, allegedly known to the injured car driver, had arrived at the scene after the collision. Public transport services in the city were at a standstill over the weekend over the issue of staff safety.

An Uzbek migrant worker has drowned and 2 others are listed in intensive care in hospital in Volzhsky after apparently being exposed to toxic fumes in a confined space accident while working at an abandoned well on a former piggery in Volgograd. Two firemen aged 60 and 25 died on Monday when a wall collapsed on 5 firemen in a building in Philadelphia where they hade been tackling a blaze in an abandoned warehouse. The collapse occurred in a furniture store adjacent to the building which fire crews had entered to check that the fire was out.

Polar bear attempted to enter Newfoundland lighthouse

A polar bear had to be shot dead by a police marksman after it threatened public safety in the community of Greenspond, Newfoundland. Staff at the Puffin Island Lighthouse reported that it had attempted to gain access to the lighthouse tower, the decision to kill the animal was taken as it subsequently moved in the direction of Greenspond where the safety of schoolchildren became a concern.
Last week in Newfoundland a polar was also shot after invading a farm area and killing stock. 5th April 2012

A New Hampshire arborist has died in an apparent crush accident while working 10 metres up a tree on premises at Parkview, Manchester. As he was cutting tree limbs, a heavy section fell and appeared to pull and strain his harness, pinning him against the trunk of the tree. His death was given by an attending doctor as asphyxiation

A 3-metre deep excavation collapsed on Tuesday at a site adjacent to a house in Lachine, Montreal. Two workers were in the trench when the accident occurred, fire crews used timber and hydraulic equipment to stabilise the locus, one worker was rescued and hospitalised with a broken leg, another was missing under the debris.

A 53-year old employee of Adirondack Utility Construction has died in an accident in Middletown Springs, Vermont, during the installation of broadband internet for VTEL(Vermont Independent Telephone Company). He was preparing a MEWP(mobile elevating work platform) for work at height when the machine contacted an overhead power line. It was unclear whether he died from electrocution or a head injury when he fell backwards.

He had been raised 10 metres in a MEWP(mobile elevating work platform) to repair a disconnected overhead line. To protect himself during a thunderstorm he had opened and raised an umbrella which conducted lightning, causing him to fall from the MEWP.

An explosion occurred during the installation of a water separator at the Petrocedeno heavy oil upgrader at the Petroleos de Venezuela facility in Anzoategui, a worker died in the explosion and a colleague sustained 2nd degree burns.

Primary school girl choked to death on food

A 6-year old girl died after she choked on food during a meal break at Pentland Primary School in Billingham, efforts to resuscitate her in hospital in Stockton were unsuccessful. Cleveland Police have reported that the death was caused by choking and is not a suspicious incident.

Great white shark attacks diver

An attack by a great white shark claimed the life of a diver at the weekend, the accident occurred a mile off Stratham Beach, 150 miles south of Perth, where he was diving from a boat.
The Fisheries Department is investigating the accident, the 4th shark attack incident in Western Australia in the last 7 months.

Fatal crash at Goodwood circuit

The 27-year old driver of a Caterham-type sports car was killed in an accident during a time trail in an event organised by the Motor Sports Association at the Goodwood motor circuit at the weekend. She lost control of her car coming our of a bend and struck a tyre wall. She was transferred to Southampton General Hospital where she died of her injuries.

Rome rail collision

On Thursday evening 2 trains of Trenitalia's highspeed Frecciarossa(Red Arrow) fleet collided in Rome's Termini station, 3 passengers and 3 staff were injured. One of the trains was travelling from Milan, the other from Salerno, the speed at the moment of impact was under 20mph. The cause was not immediately clear but it was speculated that a derailment caused one of the trains to strike the other on an adjacent track.

Kent girl dies during PE lesson

A 14-year old Kent schoolgirl died during a gym class yesterday at Angley School in Cranbrook, attempts to resuscitate her at William Harvey Hospital proved unsuccessful.
Kent Police described the death as unexplained but they believed no suspicious circumstances to be connected to the incident.

Python kills owner after escaping from reptile compound

An investigation is ongoing into a fatal accident in Ushiku, 30 miles northeast of Tokyo, where a 66-year old man was killed by a 7-metre long python which bit him in the head and arms.
The family operates an exotic pet business and the deceased is believed to have been attacked by the python which escaped from a reptile compound adjacent to his house. The escaped python was found at the locus.

Diver treated for bends

A diver received recompression treatment in Craigavon following a build-up of nitrogen in a rapid ascent from a depth of 22 metres during a dive near Killary Harbour, Connemara. The Irish Coastguard arranged his transfer as a local decompression chamber was unavailable.
The length of time a diver spends decompressing during his ascent is a function of the depth of the dive and time spent there.

Surgical reattachment attempted of arm severed in conveyor

Surgeons at Cork University Hospital have been attempting to reattach the lower half of a worker's arm which was severed in accident during the loading of animal feed to a lorry at Dan O' Connor Feeds in Limerick. A conveyor system was loading the feed from a machine to the vehicle and it is speculated that the haulage contractor may have been attempting to retrieve a fallen object when his arm was severed at the elbow.

Hessen bin man injured in crush accident

A south Hessen refuse collector was badly injured on Tuesday when he became crushed under the lifting mechanism of a bin wagon as the vehicle negotiated an alley in Gross-Rohrheim, Darmstadt.
Initially he became pinned against a house wall by the reversing vehicle and then fell backwards under the lifting platform. His injuries are not believed to be life-threatening.

At the High Court in London  a former groundsman of the Castle Combe racing track has been awarded £2m plus a £250K payment followed by an annual payment of £205K over an accident which left him brain damaged when he fell from a lorry during windy conditions while gathering metal advertising hoardings at the circuit.

Fall from raised forklift led to fatal accident

At Langanside Crown Court Walter C Campbell Joinery has been fined £2K plus £1,750 in costs over a fatal accident at its premises in Ballinamallard, County Fermanagh, in February 2010 in which an electrician died in a fall from a caged platform being raised by a forklift. Investigation found that the platform had not been securely attached to the forklift and no assessment for work at height had been undertaken for the work to be completed safely.
The company pleaded guilty to a breach of Article 5 of the Health & Safety at Work Order(NI) 1978.
